Prognostic risk factors influencing survival and prediction of the mortality in patients with acute renal failure (ARF) was analyzed using acute physiologic and chronic health evaluation (APACHE)Ⅲ score. A retrospective cohort anaysis of 105 patients with a primary clinical diagnosis of ARF was performed. ARF was diagnosed when glomerular filtration rate was acutely reduced to les than 50% of normal renal function or serum creatinine level was increased above 2.0 mg/dl in person who had normal renal function. Date were collected from January 1991 to December 1994. For each patient, the patients' vital signs and laboratory tests on adission was transformed into an APACHE Ⅲ score. Common causes of ARF were septic shock(37.2%), Korean hemorrhagic fever (28%), hypovolemic state(16.2%) and obstructive nephropathy(3.6%). Of the 105 patients, survivors were 69 patients (65.7%) and nonsurvivors were 36 patients (34.3%). APACHE Ⅲ score were significantly different between survivors and nonsurvivors (p<0.0001). APACHE Ⅲ score could be used to predict the mortality of acute renal failure patients. Key Word : APACHE Ⅲ score, ARF
