        치열교정환자의 진료인식에 따른 만족과 추천에 미치는 영향

        The purpose of this study was to offer information to medical institution of specializing in orthodontic patients by surveying the influence of orthodontic patients` perception of treatment according to orthodontic treatment upon satisfaction and recommendation with orthodontic treatment, targeting patients with completion of orthodontics who have experience of having received orthodontic treatment. As a result of SAS ver. 9.1 analysis on what was surveyed from February 2010 to August, the following conclusions were obtained. In comparing perception of treatment on orthodontic treatment according to general characteristics, women compared to men were indicated to be higher (p<0.05) in inconvenience recognition (p<0.05) on orthodontic device and in inconvenience during orthodontic treatment (p<0.05). In perception of treatment on orthodontic treatment, the inconvenience of orthodontic device accounted for 79.3%, thereby having been recognized by the majority of subjects. Even the inconvenience during orthodontic treatment was being recognized by 69.8%. The orthodontic treatment expenses appeared enough to be expected by 61.6%. The frequency of visiting hospital during orthodontic treatment was indicated as saying of having been proper by 72.8%. The period of orthodontic treatment was indicated to be proper as well by 61.2%. Even understanding about treatment contents during orthodontic treatment was indicated to be understood by 62.6%. In the results of correlation among subjects` cooperation with dental treatment, satisfaction with treatment service, recommendation of this clinic, and satisfaction with the convenient facilities, the significantly positive correlation was shown (p<0.05) in all among items in each.

        Comparison between anterior segmental osteotomy versus conventional orthodontic treatment in root resorption: a radiographic study using cone-beam computed tomography

        AbstractBackground: Patients who received orthodontic treatment are likely to have apical root shortening. It appears thatexternal apical root resorption results from a combination of patient-related risk factors such as genetic influences,systemic factors, and orthodontic treatment-related factors. Regarding the fact that the anterior segmentalosteotomy (ASO) has been known for its possibility of complementing external apical root resorption and ofbuffering periodontal problems, it has been the preferred treatment. However, the studies on the efficacy of ASO inpreserving the root are not sufficient. In this study, we compared the amount of root resorption between thepatients who only received orthodontic treatment and the patients who received orthodontic treatment with ASO. Methods: This study included 28 patients (the number of incisor = 198) who received orthodontic treatment withor without ASO. We categorize them into groups A and B by the type of orthodontic treatment (group A:conventional orthodontic treatment; group B: orthodontic treatment with ASO). Cone-beam computedtomographic and cephalometric evaluations were retrospectively performed on the radiographs taken for thediagnosis of the treatment before treatment and at the end of active treatment. Results: In group B, root resorption itself and its rate both turned out to have significantly lower than those ingroup A. Also, the change of incisal angle is significantly smaller in group B than in group A. On the other hand, ingroup A, the change of incisal angle was positively correlated with the change of AP (anteroposterior) position. Ingroup B, the change of incisal angle was negatively correlated with the duration of the orthodontic treatment. Ingroup B, amount of root resorption (mm) was positively correlated with the duration of the orthodontic treatment. Conclusions: The results show lesser root resorption and shorter treatment duration with ASO than withconventional orthodontic treatment. Therefore, if the indications are accurately determined, ASO can be an effectivetreatment option when the amount of root resorption is expected to be high, especially in late adults.

        Part I. What drives Korean adults to seek orthodontic treatment: Reliability and validity of a measurement instrument for the perception of orthodontic treatment

        Objective: To develop a standardized instrument to measure the level of cognition for orthodontic treatment in adults, and verify its reliability and validity for assessing perceptions of orthodontic treatment in adults. Methods: A total of 406 adults aged 19–64 years were surveyed by an internet research system. A tool was developed through the instrument development and verification stages. The data were analyzed by correlation analysis, exploratory factor analysis, confirmatory factor analysis, and Cronbach’s α test. Results: The instrument consisted of 11 items covering four factors related to orthodontic treatment. Three items were related to general perception, four described the perception of the treatment for adults, two related to the treatment effects, and two related to the retention of orthodontic treatment. In the reliability test, Cronbach’s α was 0.845 for the 11 items. In assessments for individual components, Cronbach’s α was 0.764 for the general perception of orthodontic treatment, 0.705 for the perception of this treatment for adults, 0.707 for the effects of the treatment, and 0.701 for the retention of orthodontic treatment. Finally, a measurement instrument for the perception of orthodontic treatment in adults was designed to assess the 11 items on a four-point Likert scale. Conclusions: This study developed a standard measurement instrument for assessing the perception of orthodontic treatment in adults. The proposed instrument will enable additional studies on the influence of an adult’s perception of orthodontic treatment on the decision to undergo treatment.

        일반인의 교정치료에 대한 지식수준, 태도에 대한 연구

        Objectives : The purpose of this study was to examine the knowledge of people in general about orthodontic treatment and their attitude to that by conducting a survey in a bid to provide information on the improvement of their incorrect awareness of orthodontics and on the development of clinical orthodontic treatment. Methods : The subjects in this study were 389 people in general who resided in Seoul and Gyeonggi Province. A self-administered survey was conducted, and the findings of the study were as follows: 1. As for awareness of orthodontic treatment among the people in general by gender, the women knew better about orthodontic treatment than the men, and the gender gap was significant(p<0.05). 2. Concerning satisfaction with the shape and arrangement of the teeth by age, those who were in their 20s were most satisfied, and the teens were satisfied the least. There were significant gaps according to age (p<0.05) 3. As a result of checking the degree of their inconvenience about the use of chewing teeth by age, the people who were in their 30s felt the most inconvenience, and the gap between them and the others was significant(p<0.01). 4. As a result of asking them whether they had an intention to receive orthodontic treatment, the women had a more intention to do that than the men(p<0.001). By age, the younger respondents had a more intention, and the gaps between them and the others were significant(p<0.001). 5. As a result of analyzing their perception of the positives of orthodontics by gender, the women took a more positive view of it than the men, and their gap was significant(p<0.01). Conclusions : When orthodontic treatment or orthodontic treatment counseling is provided to orthodontic treatment patients or people in general in the field of clinical dentistry, it should be noted that the way patients and people in general look at orthodontic treatment is different according to their age and gender, and they should be informed of the positives of orthodontic treatment to step up the development of orthodontic treatment.

        측두하악장애, 골관절염 환자의 교정치료; 교정 시 고려사항

        Signs and symptoms of Temporomandibular disorder(TMD) may appear before, during, or even after orthodontic treatment. It is recommended for all patients who visit clinics for orthodontic treatment to check the presence or absence of symptoms through a simple TMD evaluation and inform them of the results in advance. Also, modifying the orthodontic treatment strategy depending on the presense of TMD is beneficial to patients. In patients with weak physiologic tolerance who initially have no signs or symptoms of TMD, signs or symptoms may develop if the temporomandibular joint complexes fail to adapt to the changes following orthodontic treatment. To this end, periodic TMD inspections should be continued throughout treatment period and appropriate medical interventions for the temporomandibular joint should be performed. In addition, patients should be informed that it may be necessary to receive TMD treatment after stopping orthodontic treatment when TMD symptoms occur during orthodontic treatment, and that orthodontic treatment can be resumed when symptoms improve.

        Study on the perception of orthodontic treatment according to age

        Objective: This questionnaire study aimed to estimate the overall frequencies of positive perception towards orthodontic treatment among adults categorized according to age, sex, and area of living, and to identify barriers or negative perceptions preventing them from receiving orthodontic treatment. Methods: The participants included 598 adults aged over 20 years (230 men and 368 women) who visited the Dental Hospital of Seoul St. Mary’s Hospital. The participants’ opinions regarding their consideration of receiving orthodontic treatment were recorded using a specially designed questionnaire. Results: The overall rate of positive perception towards orthodontic treatment was 48.5%. Compared to adults in their 20s (63.2%), those in their 40s and 50s had a lower percentage of interest in orthodontic treatment (46.2% and 45.1%, respectively; p < 0.05). Overall, women (52.2%) had a higher rate of interest than did men (42.6%; p < 0.05). The area of living had no effect on the percentage of interest. The order of priority of chief complaints differed according to age: protrusion for those in the 20s and 30s, and spacing for those in the 40s to 60s. Overall, the main reason for not seeking treatment was the treatment fee. Respondents aged over 40 considered themselves “too old” for orthodontic treatment. Conclusions: The middle-aged had a relatively high percentage of interest (above 45%) in orthodontic treatment. However, demographic characteristics were not significantly associated with the positive interest. These results highlight the need for educating the middle-aged about the limitations and possibilities of orthodontic treatment to increase its acceptance.

        치열교정 치료 환자의 특성과 기초 진단 후 단기간 내 치료 이행에 영향을 미치는 요인

        Objectives: The orthodontics of malocclusion has a lot of impact on the prevention of multiple oral diseases, the patients` psychological stability and friendly interpersonal relationships. Patients should undergo orthodontic treatment as soon as possible if they meet all the required treatment conditions, and earlier treatment leads to greater treatment effects. The purpose of this study was to examine the characteristics of the patients and the factors affecting whether or not to go through short-interval treatment within a week after the diagnostic assessment of malocclusion. Method: The subjects in this study were patients who were undergoing orthodontic treatment and their guardians at S, B and Y dental clinics, which are located in Gyeonggi Province, Daegu and Busan, respectively, and the all specialize in orthodontic treatment. They were interviewed from January 20 to February 28, 2009, by utilizing structured questioruaires, and the answer sheets from 382 respondents were analyzed, except for 18 that had inappropriate or no answers. The SPSSWIN 14.0K program was employed to analyze the collected data, and frequency analysis, descriptive statistics, crosstabs, ANOVA and multiple logistic regression analysis were utilized. In the regression analysis, whether they underwent treatment within the six months after being diagnosed was selected as a dependent variable, and the selected independent variables included the characteristics of the subjects, what put pressure on them in association with treatment, the treatment-related decision-making process, and the patients` expectations. Results: The major subject group was students (71.7%), and among them about half (45.8%) were middle and high school students. In regard to what usually put a strain on them in conjunction with their oral health, the girls were under more aesthetic pressure (p<0.05), and there was no gender gap for the other sorts of burdens. As for the post-treatment expectations, the largest group (49.3%) expected themselves to have a good set of teeth. Concerning the major reason for the delay of treatment, cost and time constraints were cited as the reasons. In the regression analysis, women (OR=2.87, 95% CI=1.27-6.50) in their, 20s (OR=8.67, 95% CI=2.07-36.26) and 30s (OR=10.88, 95% CI=1.38-85.98), as compared to the teenagers underwent more within the six months interval orthodontic treatment. In contrast, for the case of patients having multiple visits to other orthodontic dental clinics, those patients who; visited to an orthodontic dental clinic two times (OR=0.126, 95% CI=0.05-0.31), more than three times (OR=0.098, 95% CI=0.01-0.78), a higher education (college) level (OR=0.063, 95% CI=0.01-0.77), and more burdened with mastication difficulties (OR=0.301, 95% CI=0.12-0.76) underwent less short-interval treatment. Conclusion: This study attempted to determine what factors had an impact on whether to undergo short-interval treatment, and it largely hinged on the individual needs and characteristics in addition to the above-mentioned factors. Nonetheless, the findings of this study are expected to lay the groundwork for the successful application of orthodontic treatment and for boosting its effects. In the future, more sustained research efforts should be made to produce more accurate and extensive results about this field.

        치아교정 초진 환자 만족도와 교정 치료 결정요인에 관한 연구

        Objectives: This study aimed to assess the satisfaction of orthodontic patients and evaluate the factors influencing the orthodontic treatment decision. Methods: A survey was conducted targeting patients and their guardians who wanted orthodontic counseling (n = 25). The survey comprised eight questions each for pre-visit telephone consultation and visit counseling. Results: The patient satisfaction was high with the first greeting in telephone consultation but low for active guidance and polite tone. During the visit counseling, patient satisfaction was high with the waiting time and waiting environment but low for response attitude and treatment time in the final stage of additional inquiry and diagnosis stage. Logistic regression analysis revealed significant association between orthodontic treatment decision and the treatment purpose, with re-orthodontic and pediatric orthodontic treatments showing high association with orthodontic treatment. Conclusions: The environmental factors of medical institutions and politeness of medical staff during consultation with patients should be considered to improve patient satisfaction, as they have complex effects on orthodontic treatment decisions.

      • 뇌병변 장애 환자와 자폐성 장애 환자의 교정치료: 증례 보고

        장애인의 교정 치료는 환자에 따라 치료 목표와 방법이 비장애인의 교정 치료와 달라질 수 있다. 교정 치료를 시작하기 전에 환자의 교정 치료의 한계에 대한 보호자와의 충분한 의사소통이 먼저 이루어져야 하고, 교정 치료는 환자가 견딜 수 있으면서 도움이 되는 방향으로 시행되어야 한다. 또한 교정치료 시 보호자들에게 구강위생관리의 중요성을 항상 강조하고 집에서의 구강 위생 관리를 위한 교육을 반드시 시행해야 한다. Malocclusion occurs more frequently in Special Health Care Needed (SHCN) patients than those in general. As caregiver’s needs for orthodontic treatment tend to increase, the dentist should know how to decide the extent of treatment. This case report is about orthodontic treatment for two SHCN patients; one patient with cerebral palsy, and another patient with autism. A 10-year-old patient with cerebral palsy showed protrusion and rotation of maxillary anterior teeth. To resolve his chief complaints and make better oral hygiene, he underwent orthodontic treatment using micro tube appliances for 6 months. Another 11-year-old patient with autism had anterior crossbite and showed space deficiency of #13 and chronic gingivitis because of poor oral hygiene. She underwent orthodontic treatment with maxillary skeletal expander, facemask and AP expansion appliance. After 18 months we found positive overjet and ended the treatment. When giving SHCNs orthodontic treatment, the extent of treatment can be chosen according to the patient’s cooperative ability and the traits of disabilities. Before initiating orthodontic treatment, the caregivers should be aware of their limitations of the treatment. Since oral hygiene is crucial factor in every dental treatment, education of oral hygiene process for the caregivers and SHCN patient must be done before the orthodontic treatment.

        전북대학교병원 소아치과에 내원한 교정환자 보호자들의 의삭에 관한 설문조사

        최근 치의학의 눈부신 발전과 소득의 증가에 따른 일반인의 의식 변화로 외모에 대한 관심도가 높아지고 있다. 본 연구에서는 대학병원에 내원한 동기와 교정치료를 시작하게 된 동기,치료방법 및 치료기간을 파악하여,이를 토대로 보호자와 보다 적절한 의사소통을 위한 정보를 파악하고,보호자들의 교정치료에 대한 기대를 파악하고자 본 연구를 시행하였다. 이를 위해 전북대학교 치과병원 소아치과에 내원한 교정환자의 보호자 150명을 대상으로 조사 연구한 결과 다음과 같은 결론을 얻었다. 1. 본 병원의 소아치과를 찾게 된 이유 중 대학병원이 좋을 것 같아서가 52.1%. 친지나 아는 사람의 권유가 25%. 다른 치과병원의 소개가 16.7%로 나타났다. 2. 내원 전 예상하였던 치료기간 중 2년 이상이 37.5%. 12∼18개월이 12.5%로 나타났다. 3. 교정치료를 받으려는 이유 중 부모가 부정교합을 발견하고 걱정스러워서가 58.3%, 주위사람들의 지적이 12.5%로 나타났다. 4. 소아치과 내원 환자의 치료방법은 구강 내 고정성장치가 41.7%, 가철성장치가 29.2% 구외 장치가 2.1%를 차지했다. 5. 예약 후 치료를 위해 기다리는 시간 중 5∼10분이 39.6%, 15∼30분이 4.1%로 나타났으며,기다리는 시간은 어느 정도까지 괜찮다고 생각하는가에 대해 5∼10분이 60.4%, 15∼30분이 2.1%로 나타났다. Recently, in proportion to the remarkable development of dentistry and income increases it is growing more and more a concern about changed awareness in appearance, In this study, it had a grasp of the purpose for university hospital visiting, the motive of the commencement for orthodontic treatment and a method of the treatment as well as term. Based on these, the aim of this study is to keep more of the information between parent and doctors for mutual understanding and to grasp the characteristics for the needs of orthodontic treatment. In order to conduct researches, there has made a survey of 150 persons among orthodontic patients' parents who visit CBNU hospital, the pediatric dentistry. The study has found the results like these. 1. There was a question about the reason to visit CBNU hospital in the department of pediatric dentistry for orthodontic treatment. 52.1% of respondents, the survey found, were more likely to receive a good medical service. 25% of them were counselled from a relative or an acquaintance. 16.7% of them were recommended by another dental clinic. 2. There was a question about the expected orthodontic treatment period, when at first hospital visiting. 37.5% of the respondents answered that it was a 'more than 2 years', 12.5% of them said 'from 12 months to 18 months'. 3. There was a question about the reason to receive orthodontic treatment. 58.3% of the respondents, the survey found, answered the reason was parents' concern about the malocclusion of their children, 12.5% of them said a the orthodontic problem pointed out by entourages. 4. There was a question about the method of orthodontic treatment for patients who visit the department in pediatric dentistry. 41.7% of the respondents said that it was used as 'an intra-oral fixed appliance', 29.2% of them said 'an intra-oral removable appliance', 2.1% of them said 'an extra-oral appliance'. 5. There was a question about the waiting time for treatment after a dental appointment. 60.4% of the respondents said 'from 5 minutes to 10 minutes', 4.1% of them said 'from 15 minutes to 30 minutes'. There was a question about the extent of an acceptable waiting time. It was answered to 'from 5 minutes to 10 minutes' by 60.4% of them, 2.1% of them said 'from 15 minutes to 30 minutes'.
