      • Walking-Evoked Erection in Patients with Lumbar Degenerative Diseases: Eight Cases and Review of the Literature

        Study Design: Narrative review with a case illustration.Purpose: The purpose of this study was to evaluate the pathogenesis and outcome of therapy for walking-evoked erection in patients with lumbar degenerative diseases.Overview of Literature: Cauda equina compression due to lumbar degenerative diseases rarely cause a walking-evoked erection; however, no review has been undertaken of walking-evoked erection in patients with lumbar degenerative diseases.Methods: A total of 1,570 male patients with lumbar degenerative diseases, who underwent surgery between April 2003 and June 2017, were evaluated; from these patients, participants with walking-evoked erection were selected. Preoperative clinical data of walking-evoked erection, paresthesia, and bladder and bowel function were assessed. In our study, the neurological status and the erectile function of each participant were retrospectively evaluated before and after surgery using the Japanese Orthopedic Association score and the Overactive Bladder Symptom Score.Results: Among the 1,570 male patients screened in our department, eight patients (0.51%, 8/1,570) presented with walking-evoked erection accompanied by cauda equina symptoms. In six of the patients, the erectile symptoms were associated with paresthesia in the genitalia or perianal region. Of the six patients evaluated for bladder dysfunction, all were diagnosed with prostatic hyperplasia, while four were diagnosed with an overactive bladder. In all patients, walking-evoked erection disappeared entirely after surgery.Conclusions: This study comprises the first review of walking-evoked erection in patients with lumbar degenerative diseases. We speculate that sensory input, such as paresthesia in the genitalia or perianal region stimulates the pelvic or perineal nerves through the pudendal nerve and induces reflexogenic erections.

        대공간 건축물 Erection 공법에 관한 사례 조사 연구

        최근 산업 발전과 더불어 대공간 건축물의 수요가 급증하고 있다. 이 분야 선진 기술은 지간 300m 이상의 대공간 건축물 실현도 가능하게 하고 있다. 대공간 구조에는 쉘구조, 스페이스 프레임 구조, 막구조, 케이블구조 등이 있다. 대공간 건축물은 기둥 없이 넓은 공간을 확보해야 하는 구조적 특성 때문에 설계초기 단계에서부터 시공문제를 병행하여 검토할 필요가 있다. 대공간 건축물 시공에 있어서 erection 공법은 공사비용, 공사기간 그리고 안전성 등에 큰 영향을 주는 것으로 알려져 있다. 대공간 건축물 erection 공법은 현장 여건 및 제반 조건에 따라서 그 수를 헤아릴 수 없을 정도로 많고 다양하지만, 대표적인 공법으로는 Element 방식, Block 방식, Sliding 방식, Lift-up 방식 그리고 복합방식 등이 있다. 본 연구에서는 기존 대공간 건축물을 대상으로 시공당시 적용한 erection공법을 조사하여 이것을 규모별, span별, 층고별, 구조형식별로 분석 및 검토하여 향후 대공간 건축물의 효율적 erection 공법 개발을 위한 기초 자료를 제공하는데 그 목적이 있다. Recently, the demand of the large span structures has been increasing. The large span structures include such a large scaled structures such as: the shell structure, the space frame structure, the membrane structure and the cable structure, etc. The large span structures are supposed to be confirmed and issued carefully at the initial process of the design besides the construction engineering aspects because of the structural specific cause that should solve and accomodate those large and wide space without columns. In the field of the large span structure construction, the erection construction method has been regarded as a major affected aspects on the construction cost, construction term, and stability. In the field of the large span structure construction, there are various construction method and system could be applied depends on the condition of the construction site and other circumstances such a major construction method as: the element method, the block method, the sliding method, the lift-up method and complexed method, etc. In this study, as the case study of the erection construction method of the large span structures, after survey and study that those existing large span structures construction cases which had applied and adopted the election construction method and analysis and classify into the Uoups by the size, span, ceiling height, structural system in odor to supply and suggest the data for the enhancement and development in the field of the erection construction method as a efficient structural solution of the large span structure construction.

        Flutter stability of a long-span suspension bridge during erection

        The flutter stability of long-span suspension bridges during erection can be more problematic and more susceptible to be influenced by many factors than in the final state. As described in this paper, numerical flutter stability analyses were performed for the construction process of Zhongdu Bridge over Yangtze River using the commercial FE package ANSYS. The effect of the initial wind attack angle, the sequence of deck erection, the stiffness reduction of stiffening girders, the structural damping, and the cross cables are discussed in detail. It was found that the non-symmetrical sequence of deck erection was confirmed to be aerodynamically favourable for the deck erection of long-span suspension bridges and the best erection sequence should be investigated in the design phase. While the initial wind attack angle of -3o is advantageous for the aerodynamic stability, +3o is disadvantageous compared with the initial wind attack angle of 0o during the deck erection. The stiffness reduction of the stiffening girders has a slight effect on the flutter wind speed of the suspension bridge during erection, but structural damping has a great impact on it, especially for the early erection stages.

        Purpose: Because studies about the control of penile erection at the entral nervous system was dependent on the animal experiments, establishment of measuring penile erection is important to evaluate the effects changes at the central nervous system on the penile erection. Intracavernous pressure measurement in rats has been introduced as a suitable index for the evaluation of penile erection but this method is invasive and technically difficult. So, we established the intracavernous pressure measurement in rats as the experimental index for penile erection. Materials and Methods: 42 male adult Sprague-Dawley rats (250-300gm) were divided to saline or papaverine treated group. Rats were placed on a heating table to maintain their body temparature and anesthetized with 50mg/kg of pentobarbital sodium by intraperitoneal injection. A cannula was inserted to the left carotid artery to measure systemic blood pressure. Saline (0.05, 0.1ml), papaverine(0.05, 0.1, 0.25, 0.5, 1.0, 2.0mg) were injected intracavernously via 26 guage needle filled with saline or papaverine which connected to pressure transducer and polygraph was inserted into the corpus cavernosum on one side to inject the drug and to measure intracavernous presure. Results: Properly executed insertion and intracavernous administration produced a instantaneous but transient rise in intracavernous pressure that substantially stabilized and maintained at 5.4±0.4mmHg. Intracavernous pressure and duration of penile blood flow following intracavernous injection of papaverine are increased comared to those of saline. Conclusions: Monitoring intracavernous pressure in rats represents a suitable index for the evaluation of penile erection in small laboratory animals.

        Smooth Muscle Relaxation by the Herbal Medicine Ssanghwatang associated with Nitric Oxide Synthase Activation and Nitric Oxide Production

        Ssanghwatang (SHT) has been known to prove effective in the treatment for erectile dysfunction (ED), and its modified formula is widely used in clinical practice. However, its fundamental mechanism of action is not clearly known. It is well known that endothelial cells can achieve the relaxation of vascular smooth muscles by the release of nitric oxide (NO). NO is synthesized by the enzyme NO synthase (NOS) from L-arginine and oxygen. It is widely accepted that NO plays an important role in the relaxation of corpus cavernous smooth muscle and vasculature. In addition, in terms of the penile erection, the NO/cGMP pathway is more potent than the PCE1/cAMP pathway. The main purpose of the present study was to investigate the mechanism of the erectile effects of SHT by focusing on its direct effects on corpus cavernous smooth muscle cells. We investigated the NOS activity, nitrite concentration and cGMP levels in rat corpus cavernous smooth muscle cell lines activated by SHT extracts. Furthermore, we evaluated the effect of SHT extracts on penile smooth muscle relaxation following oral administration of SHT extract powder to rats by the dosage of 1 g/kg over fifteen days. As a result, we found that SHT stimulated NO release. NOS activity and cGMP levels were increased by SHT respectively. Furthermore, SHT relaxed the corpus cavernous smooth muscle. These results are consistent with the concept that penile erection by SHT is carried out through the NO/cGMP pathway. In conclusion, the present study shows that SHT increases the NOS activity, synthesizes NO and augments the cGMP, which mediates penile erection. Further determination of the SHT mechanism related with the NO/cGMP pathway strongly indicates that SHT can be used as a remedy for erectile impotence.
