        장루보유자의 자가 간호정도 및 문제점에 관한 연구

        This study was conducted to confirm the degree of self care and problems in patients with ostomy. The subjects were 80 patients with ostomy who were visiting to OPD in K University Medical Center and were members in the Ostomy Association of Korea. Data were collected from August, 2004 to September. The research tools were a self care measure for ostomy patient developed by Park (1996) and a problem measure for ostomy patient developed by Lee (1996). Data were analyzed by t-test, ANOVA, and Pearson`s correlation. The results were as following: 1. Male (65.8%) and female (34.2%) had the ostomy in the most. Age was the most 61 years old and over. 68.5% of the subjects had managed the ostomy by themselves. 2. The mean score of self care in ostomy was 3.57 score. Hygiene of skin care around ostomy in the behavior of self care was the highest as mean 4.47 score. 3. The problems were as following: 1) expense 2) a decline on Activity of Daily Living 3) discomfort in travel 4) worry about ostomy 5) trouble of workplace 4. In the relationship between self care and religion there was significant difference in religion(t=2.727, P=.008). 5. The relationship between self care and problem with ostomy was found to have statistically significant negative correlation(r=-.237, P=.041).

        노인 장루보유자의 영양상태 관련 요인

        Purpose: The aim of this study was to identify nutritional status of older adults receiving ostomy care and to determine risk factors related to malnutrition Methods: The participants were 122 older adults with an ostomy. Data collection was conducted using a structured questionnaire. Collected data were analyzed using descriptive statistics, x2 test, Fisher's exact test, and logistic regression with the SPSS 22.0 program. Results: Results from Mini Nutritional Assessment showed that for nutritional status, 45.9% of respondents had good nutritional status, 32.0% were at risk for malnutrition and 22.1% were considered malnourished. Factors related to nutritional status were occupation (p=.005), economic status (p<.001), stoma retention period (p=.002), and ostomy care provider (p=.005). The reason for ostomy surgery was the most important determinant of malnutrition (odds ratio 3.27; 95% CI, 1.13~9.49). An additional predictive patient factor was ostomy care provider (OR, 3.07; 95% CI, 1.08~8.76). Conclusion: These results indicate that risk factors related to malnutrition in older patients are the reason for the ostomy surgery and the ostomy care provider. Therefore, in developing nursing education programs for older adults with an ostomy consideration must be given to the characteristics and problems of malnutrition and nutrition interventions.

        Colorectal Cancer Survivors' Inner Strength, Multiple Identities, and Quality of Life by Gender and Ostomy Presence: A Cross-Sectional Study

        Purpose: This study investigated differences in inner strength, multiple identities, and quality of life among colorectal cancer survivors, considering gender and the presence of an ostomy. It also focused on identifying factors that influenced their quality of life. Methods: In this cross-sectional study, 170 colorectal cancer survivors were recruited. Inner strength, multiple identities, and quality of life were assessed through an online survey. Within each subgroup, comparisons were made in two ways: (a) between women and men; and (b) between ostomy and non-ostomy groups. Results: The quality of life for colorectal cancer survivors was higher among men than women. For inner strength, men reported higher levels of anguish and searching, whereas women showed higher levels of connectedness. The quality of life was higher in the non-ostomy group than the ostomy group. However, the non-ostomy group had a higher level of anguish and searching than the ostomy group. Inner strength emerged as the most powerful factor influencing quality of life after adjusting for age and gender. Conclusion: This study emphasizes the significance of inner strength in colorectal cancer survivors. It provides a deeper understanding of quality of life in colorectal cancer survivors by examining factors that influence it and considering how these effects differ based on gender and the presence of an ostomy. To improve the quality of life of colorectal cancer survivors, it is essential to comprehend the roles of factors such as gender and ostomy and develop individualized interventions tailored to their specific characteristics.

        장루보유자의 불편함

        Purpose: The purpose of this study was to explore and describe the distress of the patients with ostomy. Methods: A descriptive research design was adopted for data collection and analysis. Six patients with ostomy participated in the study. The consent from the participants was obtained for ethical protection. Data were collected from July 31, 2009 to January 10, 2010 using in-depth interview technique. Krippendorff’s content analysis method was utilized for data analysis. Results: Eight categories and 26 themes were extracted from the data which illustrated the lived distress of the patients with ostomy. The categories were “do not have any freedom to eat whatever I wish to eat”, “uncontrolled defecation”, “have to live through tough situation”, “keep stoma in suspense”, “disgusting ostomy”, “can not live with comfort”, “easily take a pessimistic view”, and “see how the wind blows in daily family life”. Conclusion: Ostomates were affected by the distress of having an ostomy in their physical, psychological, social and spiritual life. Individualized and continued nursing education program has to be developed in hospital and community settings in order to improve the quality of life of the ostomates.

        노인 장루보유자의 장루문제가 돌봄 가족의 우울에 미치는 영향: 부양부담감을 통한 지역사회 자원 활용의 조절된 매개효과

        본 연구에서는 노인 장루보유자의 장루관련 문제가 돌봄 가족의 우울에 미치는 영향에 대해 살펴보고, 지역사회 자원의 부양부담감을 통한 조절된 매개효과에 대해 살펴보았다. 서울 시내 S대학교 병원에서 65세 이상의 장루 보유자를 돌보는 가족 125명을 대상으로 설문을 진행하였다. 회귀분석 결과 장루관련 문제는 부양부담감에는 영향을 미치지만 우울에는 직접적인 관련이 없는 것으로 나타났다. 부양부담감은 우울에 유의미한 영향을 미치며, 장루관련 문제와 우울의 관계에서 완전 매개효과가 있었다. 한편 지역사회자원을 공식적 자원과 비공식적 자원으로 살펴보았을 때, 공식적 자원활용의 조절된 매개효과도 발견되었는데, 이는 장루관련 문제와 부양부담감의 부적 관계를 약화시키는 효과가 있는 것으로 나타났다. 본 연구는 지역사회 기반의 공식적 자원 활용을 통한 정보 및 지지의 제공이 노인 장루보유자를 돌보는 가족의 부양부담감 및 우울감을 예방하기 위한 핵심적인 역할임을 제시하였다. The purpose of the study was to examine the relationship between older adults’ ostomy-related problems and depression among caregivers. We also examined the moderated mediating effect of ostomy-related problems and the use of formal and informal community resources on family burden and depression among caregivers. Survey data from 125 family caregivers of older adults with ostomies were collected from a university hospital in Seoul, South Korea. The results showed that ostomy-related problems were significantly related to the burdens of support, but not on depression. The total indirect effect was significant, indicating that family burden are fully mediated in the relationship between ostomy-related problems and depression. In addition, this indirect effect decreased with the increased use of formal community resources. For individuals who receive support and information from formal community-based organizations, the severity of depression is lower even if they handle more ostomy-related problems. Our findings suggest that community-based organizations need to provide information on medical care and focus to reduce caregivers’ perceived levels of economic and psychological burdens.

        장루보유자의 외상 후 스트레스 장애가 삶의 질에 미치는 영향: 회복탄력성의 매개효과를 중심으로

        Purpose: This study was performed to identify the mediating effect of resilience in the relationship between post-traumatic stress disorder (PTSD) and quality of life (QoL) among patients with ostomy. Methods: A crosssectional survey design was used. A convenience sample of 150 patients with ostomy was recruited from three hospitals located in Gwangju and Chonnam province in Korea. Data were collected by self-report questionnaires and included demographics as well as measures of PTSD, resilience, and QoL. Data were analyzed with descriptive statistics, Pearson correlation coefficients, independent t-test, One-way ANOVA, and linear regression using the SPSS 24.0 program. Results: There were significant negative relationships between PTSD and QoL (r=-.30, p<.001) and between PTSD and resilience (r=-.57, p<.001). Resilience showed a partial mediating effect (β=.39, p<.001) between PTSD and QoL (Z=-3.12, p<.001). Conclusion: This study findings suggest the importance of reducing PTSD scores and improving resilience among persons with ostomy which will possibly increase their QoL. Nursing interventions including counseling or education to improve psychological resilience might help better manage PTSD among patients with ostomy.

        장루를 보유한 대장암 여성의 자아존중감, 성의사소통이 성만족에 미치는 영향

        Purpose: The purpose of this study was to identify the influence of self-esteem and sexual communication on sexual satisfaction among female colorectal cancer patients with ostomy. Methods: A cross-sectional descriptive study was conducted using self-reported online questionnaires. The participants were 85 women with colorectal cancer who had undergone ostomy formation surgery. Data were analyzed using descriptive statistics, the t-test, analysis of variance, the Scheffé test, correlation coefficients, and hierarchical multiple regression with SPSS version 26.0. Results: The mean sexual satisfaction score was 2.79±0.73 (range, 0~5). There was no significant relationship between self-esteem and sexual satisfaction. Sexual communication had a significant correlation with sexual satisfaction (r=.83, p<.001) and was a strong predictor of sexual satisfaction (β=.83, p<.001). Conclusion: The findings of this study indicate that colorectal cancer patients with ostomy experience low sexual satisfaction and that sexual communication is a meaningful factor for sexual satisfaction. Accordingly, nurses need to factor in sexual issues when caring for patients, and should encourage both patients and their partners to participate in education related to sexual health. It will also be helpful to inform patients about the importance of sexual communication with their partners as a sustainable intervention.

      • 단순 골반 골절된 장루술 환자 치험 1례

        Objective : The objective of this study is to report the effectiveness of conservative treatment on simple pelvic fracture with ostomy. Methods : A female patient 65 years old with ostomy, who were diagnosed as simple pelvic fracture by X-ray, were treated with conservative treatment by herbal medication with acupuncture and physical therapy. We evaluated the effectiveness by checking X-ray, Digital Infrared Thermal Imaging(D.I.T.I.). Results : The symptoms of the patient got improved and X-ray showed the fracture were healing. Conclusions : Conservative korean medical treatment can be effectively used for a patient with simple pelvic fracture and ostomy.

        장루를 갖고 산다는 것: 현상학적 연구

        Purpose: This study was to explore and describe their experience of living with a colostomy. Methods: The data were collected by in-depth interviews with eight patients with colorectal cancer. The individual interviews lasted from 45 minutes to one hour each time, up to two times. The main questions were "How do you cope with the distress related to your colostomy?" The interview data were analyzed using Colaizzi's phenomenological method. Results: Of the eight participants, five were men and three were women. Mean age was 60.6 years. Three theme clusters were identified; ‘overcoming adversity of ostomy', and’adjustment to ostomy' and ‘new life through self-transcendence'. Conclusion: These results showed that participants try to overcome their negative images and distress of having an ostomy by themselves, or with support from neighbors, families, religious persons or health professionals. The results from this study can help nurses to develop effective colostomy-caring interventions for patients living with a colostomy.

        The Influence of Nutritional Assessment on the Outcome of Ostomy Takedown

        Purpose: Ostomy takedown is often considered a simple procedure without intention; however, it is associated with significant morbidity. This study is designed to evaluate factors predicting postoperative complications in the ostomy takedown in view of metabolism and nutrition. Methods: A retrospective, institutional review-board-approved study was performed to identify all patients undergoing takedown of an ostomy from 2004 to 2010. Results: Of all patients (150), 48 patients (32%; male, 31; female, 17) had complications. Takedown of an end-type ostomy showed a high complication rate; complications occurred in 55.9% of end-type ostomies and 15.7% of loop ostomies (P <0.001). Severe adhesion was also related to a high rate of overall complication (41.3%) (P = 0.024). In preoperative work-up,ostomy type was not significantly associated with malnutrition status. However, postoperatively severe malnutrition level (albumin < 2.8 mg/dL) was statistically significant in increasing the risk of complications (72.7%, P = 0.015). In particular,a significant postoperative decrease in albumin (> 1.3 mg/dL) was associated with postoperative complications, particularly surgical site infection (SSI). Marked weight loss such as body mass index downgrading may be associated with the development of complications. Conclusion: A temporary ostomy may not essentially result in severe malnutrition. However, a postoperative significant decrease in the albumin concentration is an independent risk factor for the development of SSI and complications.
