        가성부갑상선기능저하증 환아의 구강내 증상에 관한 증례보고

        부갑상선기능저하증은 혈장내 칼슘 농도에 반응해 분비되는 부갑상선 호르몬의 분비가 비정상적으로 감소된 상태를 의미한다.이에 반해 가성부갑상선기능저하증은 부갑상선의 기능은 정상이나 뇨세관과 파골세포등의 말단기관들이 부갑상선 호르몬에 반응하지 않는 질환이며,원인은 이들 호르몬이 결합하는 수용성 성분인 Guanin protein을 결정하는 GS gene의 돌연변이에 의한다. 부갑상선기능저하증은 평균8-9세에 발병하며 대부분이 20세 이전에 호발하게 되는데,이 질환에 이환된 경우 특징적으로 뇌,호르몬 기관,연골세포등에 존재하는 GS gene의 돌연변이로 인해 성장 및 발육부전,둥근 얼굴,비만,허리만곡증,외골증등이 생기며,조기 골단 폐쇄로 인한 중수골 및 중족골의 왜소증이 나타난다.그리고 공통적으로 갑상선 자극 호르몬,생식 호르몬,glucagon등의 다수 호르몬에 대해서도 반응하지 않는다. 이들의 치과적 특징은,법량질의 형성부전과 맹출 지연이 가장 일반적이고 치배의 무형성,치배결손,짧고 이형성된 치근과 넓어진 치수강,왜소치,치수내 석회화,부정교합 등도 자주 보고되고 있다. 가상부갑상선기능저하증으로 진단된 본 증례 환아의 구강내 소견으로는 전반적으로 짧고 미발육된 영구치 치근과 소구치 및 대구치의 맹출 지연과 미맹출,제 1,2 소구치와 제 2 대구치이 형성 부전양상이 보이고 있다. 이에 저자는 연세대학교 치과병원 소아치과에 내원한 가상부갑상선기능저하증 환자를 경험하였기에 문헌고찰과 함께 보고한는 바이다. Hypoparathyroidism has abnomally decreased secretion of parathyroid hormon which responds to the blood calcium level.Whereas, Pseudohypoparathyroidism has normal activity of thyroid hormon,but end-organs,such as uninary tract and osteocalst,do not respond to parathyroid hormon.The cause of this disease is due to the mutation of Guanine stimulating(Gs)protein regulating Gs gene,which is the receptor to this hormon. Pseudohypoparathyroidism is usually noted before 20 years old average of 8-9 years old.The clinical features of this disease includes delayed growth and development,round face,obesity,soft tissue calcification,ectopic ossification,shortening of metacarpals and metatarsals by epiphyseal closure in advance of age. The mutation of Gs gene which are found in brain,endocrine organs,and chondrocytes is the cause of those features. Reaction to Glucagon,gonadal hormon,and thyroid stimulating hormon is not expected in both cases. The common dental manifestations include enamel hypoplasia,delayed eruption,agenesis of tooth,hypodotia,dysplastic short roots,widened pulpal space,microdontia,intrapulpal calcification,and malocclusion are also often reported. This case which is diagnosed to Pseudohypoparathyroidism showed short and under developed root of permanent teeth,delayed eruption,and non-eruption of premolars and molars.And morphogenesis imperfecta of first and second premolars were also found.

        상아질 접착제를 사용한 광중합 복합레진과 Glass lonomer Cement의 전단 결합력에 관한 연구

        The purpose of this study was to compare shear bond strength of composite resin using several dentin bonding agents and light cured glass ionomer cement(Fuji Ⅱ LC). 40 Bovine primary anterior teeth were used for this experiment.Labial surface of teeth were flattened.It were divided into four groups.Each group was composed of 10 teeth.The material used for this experiment were Scotchbond Multipurpose-Z-100,Allbond 2-Aelitefil,Gluma-Pekalux,light and cured glass ionomer cement(Fuji Ⅱ LC). Each of the materials was applied to the exposed surface of 10 teeth by insertion into a cylindrical shaped matrix which is 3mm diamiter and 3mm in height. The completed specimens were stored at 37℃ under 100% humidity for 24 hours:the shear bond strength of material to detin surface were measured with INSTRON universal testing machine. The results were as follows: 1.Shear bond strength to detin surface increased in oder of light cured glass ionomer cement(Fuji Ⅱ LC),Gluma,Allbond 2,Scotchbond Multipurpose. 2.Between shear bond strength of light cured glass ionomer cement(Fuji Ⅱ LC) and Allbond 2,there was statistical significace(P<0.05). 3.Between shear bond strength of light cured glass ionomer cement(Fuji Ⅱ LC) and Scotchbond Multipurpose,between shear bond strength of Gluma and Scotchbond Multipurpose,there was statistical significace(P<0.01). The shear bond strength of detin bonding agents were higher than light cured glass ionomer cement.The reason is that material and quality of detin bonding agent were enhanced.Further investigation is necessary to improve shear bond strength of light cured glass ionomer cement.

        도재시편을 투과한 가시광선이 복합레진 중합에 미치는 영향에 관한 실험적 연구

        The purpose of this study was to evaluate the effect of the of the visible through the porcelain disc on the curing of the composite resin. Round porcelain discs 10mm in diameter were processed in two thicknesses; 0.5mm and 1mm and black plastic molds 6.5mm in inner diameter were prepared in three heights;1mm,2mm and 3mm. A mold was placed on the glass plate and pressure filled with composite resin(hybrid type CHOICE porcelain adhesive paste,shade Al,Bisco Dental Product Co.). In experimental group,a celluloid strip was placed over the composite resin and porcelain disc was placed on the top.The composite resin specimens were cured with visible light curing unit using exposure time of 30,60 and 90 seconds. In control group,only celluloid strip was placed over the composite resin and cured with visible light cured unit using given three different exposure time. The variables of the porcelain disc thickness,the exposure time and specimen height produced total 81 specimens. After the visible light exposed,the surface Knoop hardness number of each composite resin specimen was measured on both sides(top and bottom). Results of this study were as follows; 1.The porcelain disc diminished the degree of the visible light transmission and as the thickness of porcelain disc increased,the surface hardness and curing depth of the composite resin specimen decreased with statistical significance. 2.As the exposure time of visible light increased,the surface hardness and curing depth of the composite resin specimen increased with statistical significance. 3.Even though exposure time of visible light was 90 seconds,the bottom surface of 3mm height composite resin specimen was not cured. 3.As the exposure time of visible light increased,the difference of top surface hardness and 1mm bottom surface hardness was reduced. 4.In clinical situation of the porcelain laminate technique,it is supposed that at least 90 seconds of visible light exposure time is necessary to obtain the adequate curing of the composite resin. 5. In clinical situation of the porcelain laminate technique, it is supposed that at least 90 seconds of visible light exposure time is necessary to obtain the adequate curing of the composite resin.

        Pulse Oximeter를 이용한 치수생활력측정

        치수 생활력 검사시 전통적인 방법으로 전기치수검사나 온도변화검사 등이 있다. EPT와 ice test 즉, 전기치수검사와 온도변화검사는 치아의 신경학적 반응에 의해 치아의 실활여부를 판단하는 방법으로 환자가 아동일 때는 정확한 반응을 얻기가 어렵고, 환자의 주관적 반응을 판단해야 하므로 객관적이지 못하고, 소아환자에게 좋지 못한 자극을 주어 행동 조절의 문제를 일으키며, 거짓 양성반응과 거짓 음성반응이 나올 수 있다는 등의 한계가 있다. 최근에는 전통적인 방법의 한계를 극복하려는 시도에서 혈관의 보전성을 평가하는 방법인 laser doppler flowmetry와 pulse oximeter를 이용하는 방법이 소개되고 있다. Pulse oximeter의 원리는 두 가지 종류의 파장의 빛을 귀, 손가락 등 생체의 말단에 투과시켜 발산된 빛과 감지된 빛간의 두 파장의 흡수비로 산소포화 정도를 알아내는 것으로서 이에 착안하여 또 하나의 생체 말단인 치아에 이를 적용하여 치아의 실활 여부를 판단하는 것이다. 이 보고에서는 치수 생활력 검사시 pulse oximeter의 사용 가능성에 대해 검증하고 이의 임상적용에 대해 기초적 자료를 제공하고자 함을 목적으로 했는데 생활치에서는 평균 96.3%의 산소 포화도를 실활치에서는 평균 0.0%의 산소 포화도를 얻어냄으로서 pulse oximeter가 치아의 실활여부 판단에 있어 유용한 진단도구로서의 가치가 있다는 것이 밝혀졌다. Traditionally, EPT and thermal tests were used as diagnostic methods for pulp vitality test. The thermal and electrical stimulation tests are the methods to determine the vitality of a tooth based on its neuronal response. These have certain limitations, one of them is the difficulty of approaching the correct result in case of treatment of children. The reason is management problem caused by the unpleasant stimulation. Also, the response from patients are not objective, and false positive or false negative could be happened. Recently, laser doppler flowmetry and pulse oximeter which evaluate vascular integrity are introduced-in an effort of overcoming to limitation of traditional methods. The principle of pulse oximeter is to find out level of oxygen saturation by ratio of the two pulses between emitted light and detected light penetrating them to the termination of body, such as ears or fingers. From this point of view, it can be applied to a tooth to determine its vitality. The objective of this study lies mainly on varifying pulse oximeter as a method of determining tooth vitality and providing basic data of its clinical implementation. The result of the research showed that level of oxygen saturation in vital teeth was average of 96.3% and 0.0% in pulpless teeth. As a comprehensive result, pulse oximeter could be an useful diagnostic equipment in determining of tooth vitality.

        백혈병 아동의 구강소견 : 증례보고

        Leukemia is a malignant neoplasm of blood forming organs that accompanies oral manifestations and has the highest prevalence among the neoplasms of childhood. The author reports the following conclusions after clinical and radiographic examination of leukemic chidren who visited the Yonsei Dental Cooege; 1. Acute lymphoblastic leukemia has the better prognosis and favorable oral signs than acute myeloblastic leukemia. 2. Oral signs are gingival enlargement, gingval bleeding, ulcerative lesions of oral mucosa, lymphadenopathy, mandibular pain and paralysis. 3. On the radiographs, loss of developing crypt, periapical radiolucency, displacement of tooth germ, and loss of lamina dura are seen. 4. During the remission period, recovery of clinical and intrabony lesions can be found. 5. According to the radiologic examination, the most affected teeth are posterior teeth. 6. Leukemia is not only dangerous disease by itself but also highly dangerous chemothermpeutic procedure so consultation with the physician before the treatment is requred.

        Analysis of cholesterol concentration based on training type presents high in Walking group at 10min but low at recovery 30min. HDL-C Concentration based on training type presents higher at training 10min and recovery 10min than that of jogging group. LDL-C Concentration based on training type presents higher in walking group during stability period but presents high in jogging group at training 30min. However, jogging group presents high at recovery 10min.

        Peripheral odontogenic fibroma에 관한 증례보고

        The peripheral odontogenic fibroma (WHO type) is a relatively rare and characteristically benign and unencapsulated, exophytic gingival mass of fibrous connective. Odontogenic epithelium is found within the gingival mass, but usually appears to play a minor role when compared to the fibrous component. The connective tissue is ranged from markedly cellular to relatively acellular and well collagenized. but the connective tissue in this case report appears less cellular. Peripheral odontogenic fibroma must be differentiated histologically from peripheral ossifying fibroma, Peripheral cemental epithelial odontogenic tumor and Peripheral ameloblastoma. The author reports the following conclusions after clinical and radiological examination, excisional biopsy and reviewing literatures. 1. Peripheral odontogenic fibroma is rare lesion and frequently occurs in interdental papila as a form of fibroblastic connective tissue including odontogenic epithelium within the lesion. 2. Peripheral odontogenic fibroma must be differentially diagnosed with Peripheral ossifying fibroma by including less cellular connective tissue, odontogenic epithelium and dysplastic dentin. 3. Treatment consists of surgical excision including removal of alveolar bone which is eroded under the lesion.

        유구치 아말감 수복물에서 레진 접착성 이장재의 미세 변연 누출에 관한 비교 연구

        The purpose of this study was to compare the compare the marginal leakge of resin liner which was used as a liner to amalgam restoration. Control group composed of no liner group,copalite group and experimental group composed of All-bond 2,Vitrebond,Superbond D-liner applied groups were evaluated. CI.V preparetion with a size of 3×2×1.5mm on 70 extracted primary molars were made and applied appropriate liners for each groups. After amalgam filling and polishing,polished specimens underwent temperature changed from 5℃ to 55℃ a thousand times. After thermocycling,specimens were placed in 2% methylene blue dye solution for 72 hours in an incubator set at 37℃. The teeth were sectioned buccolingually and the degee of dye penetration was observed with a spectroscope.(×30) The following results were obtained. 1.Vitrebond,All-bond 2,Superbond D-liner group exhibited a statiscally significant lower degee of dye penetration than no liner and copalite group.(Kruskal-Wallis analysis,P<0.05) 2.No liner group and compalite group exhibited a similar degee of dye penetration.(Mann-Whitney analysis,P>0.05) 3.All-bond 2 group exhibited a statiscally significant lower degee of dye penetration than Vitrebond group,(Mann-Whitney analysis,P<0.05) Superbond D-liner group also exhibited a lower degee of dye penetration than Vitrebond group but was statiscally insignificant.(Mann-Whitney analysis,P>0.05)

Analysis of heart rate based on training type presents high during the training but is reduced recovery period. In the result Exercise prescription programs for the non-insulin-depentent diabetic patients are required to maintain their Blood pressure and heart rate.

        전산화 단층촬영술을 이용한 악하선 타석증의 진단

        Sialolithiasis is the formation of calcareous concretions within ductal system of a major or minor salivary gland. They are formed by deposition of calcium salts around a central nidus which may consist of desquamated epithelial cells, bacteria, foreign bodies, or products of bacterial decomposition. An ll-year-old boy complained of pain during meals and intermittent mild swelling in the right submandibular region. Although it was not detected in true occlusal radiograph. panoramic radiograph showed a round radiopaque mass 3mm in diameter. Computed tomography (CT) was taken for locating the stone and 3-dimensional reconstruction was per-formed . Under general anesthesia, sialoadenectomy was done through extraoral approach. diagnosis of submandibular sialolithiasis using high-resolution CT with reconstructions was helpful for surgical decisions, namely radical removal of the submandibular gland and its duct.
