Unicornuate uterus is caused by a failure of one Mullerian duct to develop.It is linked to an increase in obstetrical complications such as early miscarriages, ectopic pregnancy and preterm labor. Despite the association of ectopic ovaries and unicornuate uterus, ectopic ovaries are reported only sporadically. We introduce first case of ectopic pregnancy arising at a distal, fimbriated end of the fallopian tube, which was developed adjacent to the undescended ectopic ovary in unicornuate uterus. PI: A 32- year-old woman admitted emergency room of our clinic due to severe lower abdominal pain. Her last menstrual period was 9 weeks ago. Four weeks ago, she was performed dilatation and curettage at local clinic due to no G-sac in uterus and slowly increasing ß-human chorionic gonadotropin (ß-hCG). After dilatation and curettage, her ß-hCG was continuously increased from 2,400 mIU/mL to 2,800 mIU/mL. So, she was injected methotrexate (MTX) 50mg IM. One week after MTX injection, her ß-hCG was decreased to 1,700 mIU/mL. Her vital sign was stable. Her ß-hCG was 386 mIU/mL. Ultrasonogram: Fluid collection of anterior cul-de sac (CDS) was 70 X 50mm, posterior CDS was 30 X 26mm. The endometrial thickness was 5mm, and no G-sac. Laparoscopic Findings: Unicornuate uterus, about 500ml hemoperitoneum, left side salpinx and ovary was intact. Right side salpinx was seen, but no right side fimbriae and ovary. No ectopic mass in pelvic cavity. High up in the right abdomen, just below the liver, an ectopic mass could be visualized with a arising at a distal, fimbriated end of the fallopian tube, which was developed adjacent to the undescended right ectopic ovary. We removed ectopic mass. One day after operation, her ß-hCG decreased to 91 mIU/mL. She discharged. Hysterosalpingography: Unicornuate uterus, preserved patency of left salpinx and some right salpinx. MRI: Unicornuate uterus, Right ovary was seen at right inferior hepatic portion, bilateral normal kidney, double IVC.
Consensus Update for Systemic Treatment of Atopic Dermatitis

Background: In 2015, the Korean Atopic Dermatitis Association (KADA) working group published consensus guidelines for treating atopic dermatitis (AD). Objective: We aimed to provide updated consensus recommendations for systemic treatment of AD in South Korea based on recent evidence and experience. Methods: We compiled a database of references from relevant systematic reviews and guidelines on the systemic management of AD. Evidence for each statement was graded and classified based on thestrength of the recommendation. Forty-two council members from the KADA participated in three rounds of voting to establish a consensus on expert recommendations. Results: We do not recommend long-term treatment with systemic steroids forpatients with moderate-to-severe AD due to the risk of adverse effects. We recommend treatment with cyclosporine or dupilumab and selective treatment with methotrexate or azathioprine for patients with moderate-to-severe AD. We suggest treatment with antihistamines as an option for alleviating clinical symptoms of AD. We recommend selective treatment with narrowband ultraviolet B for patients with chronic moderate-to-severe AD. We do not recommend treatment with oral antibiotics for patients with moderate-to-severe AD but who have no signs of infection. We did not reach a consensus on recommendations for treatment with allergen-specific immunotherapy, probiotics, evening primrose oil, orvitamin D for patients with moderate-to-severe AD. We also recommend educational interventions and counselling for patients with AD and caregivers to improve the treatment success rate. Conclusion: We look forward to implementing a new and updated consensus of systemic therapy in controlling patients with moderate-to-severe AD.

The silkworm (Bombyx mori L.) droppings were extracted with 80% aqueous MeOH, and the concentrated extract was partitioned in succession with EtOAc, n-BuOH, and H_2O. From the EtOAc fraction, five megastigmane sesquiterpenes were isolated through repeated silica gel and ODS column chromatography. According to the results of spectroscopic data, such as NMR, MS, and IR, the chemical structures of the isolated compounds were determined as (3S,5R,8R)-3,5-dihydroxymegastigma-6,7-dien-9-one (1), (S)-dehydrovomifoliol (2), (6R,7E,9R)-9-hydroxy-4,7-megastigmadien-3-one (3), (3S,5R,6S,7E)-3,5,6-trihydroxy-7-megastigmen-9-one (4), (6R,9R)-9-hydroxy-4-megastigmen-3-one (5). Compounds 2 through 5 were isolated for the first time from silkworm droppings. GC/MS analysis indicated silkworm powder contained compound 3, and mulberry leaves contained compound 4. Compounds 1 and 5 increased the expression of heme oxygenase-1 and SIRT1 in HepG2 and HEK239 cells, respectively. Heme oxygenase-1 is considered to be an antioxidant enzyme that catabolizes heme to carbon monoxide, free iron and biliverdin, while SIRT1 is the mammalian homologue of the yeast silent information regulator (Sir)-2, which are involved in the suppression of inflammatory mediators or factors that may be used to improve atopy-related symptoms.

Six lignans, (+)-pinoresinol (1), (+)-lariciresinol (2), (+)-syringaresinol (3), syringaresinol-4-O-β-Dglucopyranoside (4), (+)-lariciresinol-4-O-β-D-glucopyranoside (5), and dehydrodiconiferylalcohol-9'-O-β-D-glucopyranoside (6), were isolated from silkworm droppings. The structures of six compounds were elucidated on the basis of 1D- and 2D-NMR spectroscopic analyses and optical rotational characteristics. Simple decompression of odontogenic cyst

Marsupialization or decompression can be performed, especially to decrease complications when the cyst size is too large or there is risk of injury to the adjacent structures. In this study, we attempt to evaluate the change in the cystic lesion after decompression with a silicone tube through dental panoramic radiographs at regular time intervals. All 7 patients, who had been diagnosed and treated for an odontogenic cyst in the jaws with decompression from January 2007 to January 2014, were included in the study. Differences between pre-operative panoramic radiographs and post-operative panoramic radiographs were analyzed for evaluating the size of each cyst, and the calculation of reduction rate was used. The average period was 426.6 days between decompression and secondary enucleation. The average reduction rate of the cyst was 67.68%. During the treatment, the average reduction rate per day was 0.178 %. Histologic examination after final enucleation showed no signs of cystic remnants. No recurrence occurred during the 2-year follow-up period. Decompression with a silicone tube causes less bone defects since it is necessary to remove only the upper bone as wide as the tube diameter. It can be considered as an effective treatment method because it causes no significant changes after healing and it can be applied easily.
