        A survey of current practices in post-polypectomy surveillance in Korea

        Background/Aims: We investigated the clinical practice patterns of post-polypectomy colonoscopic surveillance among Korean endoscopists. Methods: In a web-based survey conducted between September and November 2021, participants were asked about their preferred surveillance intervals and the patient age at which surveillance was discontinued. Adherence to the recent guidelines of the U.S. Multi-Society Task Force on Colorectal Cancer (USMSTF) was also analyzed. Results: In total, 196 endoscopists completed the survey. The most preferred first surveillance intervals were: a 5-year interval after the removal of 1-2 tubular adenomas < 10 mm; a 3-year interval after the removal of 3-10 tubular adenomas < 10 mm, adenomas ≥ 10 mm, tubulovillous or villous adenomas, ≤ 20 hyperplastic polyps < 10 mm, 1-4 sessile serrated lesions (SSLs) < 10 mm, hyperplastic polyps or SSLs ≥ 10 mm, and traditional serrated adenomas; and a 1-year interval after the removal of adenomas with high-grade dysplasia, >10 adenomas, 5-10 SSLs, and SSLs with dysplasia. In piecemeal resections of large polyps ( > 20 mm), surveillance colonoscopy was mostly preferred after 1 year for adenomas and 6 months for SSLs. The mean USMSTF guideline adherence rate was 30.7%. The largest proportion of respondents (40.8%-55.1%) discontinued the surveillance at the patient age of 80-84 years. Conclusions: A significant discrepancy was observed between the preferred post-polypectomy surveillance intervals and recent international guidelines. Individualized measures are required to increase adherence to the guidelines. (Intest Res 2024;22:186-207)

        Successful Transition from Pediatric to Adult Care in Inflammatory Bowel Disease: What is the Key?

        The incidence of pediatric-onset inflammatory bowel disease (IBD) is on the rise, accounting for up to 25% of IBD cases. Pediatric IBD often has extensive bowel involvement with aggressive and rapidly progressing behavior compared to adult IBD. Because IBD has a high morbidity rate and can have a lifelong impact, successful transition from pediatric to adult care is important to maintain the continuity of care. Furthermore, successful transition facilitates appropriate development and psychosocial well-being among patients, as well as comprehensive and harmonious healthcare delivery amongst stakeholders. However, there are various obstacles related to patients, family, providers, and organizations that interfere with successful transition. Successful transition requires a flexible and tailored plan that is made according to the patient’s developmental abilities and situation. This plan should be established through periodic interviews with the patient and family and through close collaboration with other care providers. Through a stepwise approach to the transition process, patients’ knowledge and self-management skills can be improved. After preparation for the transition is completed and the obstacles are overcome, patients can be gradually moved to adult care. Finally, successful transition can increase patients’ adherence to therapy, maintain the appropriate health status, improve patients’ self-management, and promote self-reliance among patients.

      • 파워트레인 HiL 환경을 위한 비선형 시스템 규명 접근법

        Hardware-in-the-Loop simulation allows useful environments and capabilities for controller development process in the automotive industry. It has been extensively used in the software validation of embedded powertrain control systems, and shows growing potential for the model-based calibration of engine management systems. To meet their requirements, a proper powertrain model is critical. This paper presents the application of nonlinear experimental identification which gives the powertrain model having good model fidelity and suitable for real-time computation. Nonlinear identification techniques are applied to building the intake manifold model of a gasoline engine with variable valve timing and the vacuum actuator system of a diesel engine with variable geometry turbo charger. Experimental driving data are used as inputs to identify them, and the outputs of the identified model are compared with the corresponding outputs of the experimental vehicle. The validity of nonlinear identification for powertrain HiL environment is discussed with respect to the powertrain development phase.

        개인정보보호를 위한 영상 암호화 아키텍처 연구

        영상 감시 시스템은 광범위한 영역에서 쉽게 설치되고 녹화 장치 혹은 인터넷을 통한 클라우드 저장소에 영상 정보를 관리하는 중앙 관리 방식을 사용하고 있다. 이러한 시스템의 주요한 문제점은 저장 영상의 전송 과정과 저장 대해서 객관적으로 신뢰할 수 있는 방법이 제공되지 않고 있으며, 개인정보보호를 위한 장치 유무와 별개로 모든 권한을 서비스 제공자에게 위임한 상태에서 운영하고 있다는 점이다. 본 연구에서는 공개키 기반 암호화와 블록체인 기반의 키 관리 시스템을 조합한 아키텍처를 이용하여 민감한 정보를 사용자가 안전하게 보호할 수 있는 방안을 제시한다. 제안하는 아키텍처에서는 대칭키를 사용한 블록 암호화(block-cipher) 과정을 통해 영상 정보를 암호화하고, 이때 사용하는 대칭키를 사용자의 공개키로 암호화하여 블록체인의 레저(ledger)로 기록하는 기법을 사용한다. 영상 정보를 암호화하는 과정을 블록체인 네트워크의 특성(분산, 투명성, 데이터 변조 불가)을 활용하여 개인정보 영상의 생성부터 소멸까지 사용자가 추적이 가능하도록 한다. The video surveillance service is being widely deployed around our lives and the service stores sensitive data such as video streams in the cloud over the Internet or the centralized data store in an on-premise environment. The main concerning of these services is that the user should trust the service provider how secure the video or data is stored and handled without any concrete evidence. In this paper, we proposed the approach to protecting video by PKI (public key infrastructure) with a blockchain network. The video is encrypted by a symmetric key, then the key is shared through a blockchain network with taking advantage of the PKI mechanism. Therefore, the user can ensure the sensitive data is always kept secure and traceable in its lifecycle.

        5G-MEC 기반 초저지연 고화질 영상 전송 시스템

        모바일 네트워크 및 인터넷의 발전은 물리적인 거리의 한계를 극복하고 원격지의 정보를 제공하거나 획득하는데 기여하고 있다. 그러나 영상 전송을 주요 정보 제공 수단으로 사용하는 시스템은 여전히 고대역폭과 저지연 전송을 요구하고 있으며, 전송된 영상을 기반으로 상황을 판단하고 실시간 피드백을 제공하기 위해서는 전송된 영상의 품질뿐만 아니라 데이터 신뢰성과 전송 지연시간 문제는 극복해야할 중요한 부분이다. 5세대 모바일 네트워크의 출현은 이전 세대의 기술에서 경험할 수 없었던 고대역폭과 정밀한 위치 인식 등의 특성을 제공하여, 원격 진료 및 수술, 사회 안전망을 위한 무선 원격 비디오 감시 시스템, 차량의 자율 주행 뿐만 아니라 UAV/UGV의 비가시권 제어를 실현할 수 있는 기반이 되고 있다. 또한 모바일 네트워크의 특성을 고려하여 네트워크 지연 시간을 최소화하는 Mobile Edge Computing 기술은 기존의 스마트 단말과 고가용성 서버 시스템으로 구성되던 시스템 아키텍처에 대한 변화를 요구하고 있다. 그러나 여전히 무선 구간에서 발생하는 네트워크 불확실성은 고해상도 영상을 전송할 때 영상 품질의 문제로 이어지며, 캐시를 활용한 전통적인 해결 방법은 지연 시간의 증가로 이어지게 되어 5G-MEC로 극복한 문제에 대한 근본적인 해결책이 되지 못한다. 본 연구에서는 Foward Error Correction과 Fast Retransmission을 이용하는 SRT 프로토콜을 기반으로 초저지연 고화질 영상 전송 시스템을 제안하고 각 시스템 컴포넌트를 5G-MEC의 특성을 고려하여 배치하여 4K 영상 전송시에도 종단간 지연시간을 1초 이하로 제한할 수 있음을 실험 결과로 제시하고 있다. 또한 실시간 고화질 영상 전송시 고려해야하는 요소로, 영상의 품질과 카메라-사용자 간의 최종 지연 시간 및 지연시간에 영향을 미치는 구간을 분석하고 추가적으로 개선할 수 있는 부분을 찾아 제시하도록 한다. The Internet including mobile networks is developing to overcoming the limitation of physical distance and providing or acquiring information from remote locations. However, the systems that use video as primary information require higher bandwidth for recognizing the situation in remote places more accurately through high-quality video as well as lower latency for faster interaction between devices and users. The emergence of the 5th generation mobile network provides features such as high bandwidth and precise location recognition that were not experienced in previous-generation technologies. In addition, the Mobile Edge Computing that minimizes network latency in the mobile network requires a change in the traditional system architecture that was composed of the existing smart device and high availability server system. However, even with 5G and MEC, since there is a limit to overcome the mobile network state fluctuations only by enhancing the network infrastructure, this study proposes a high-definition video streaming system in ultra-low latency based on the SRT protocol that provides Forward Error Correction and Fast Retransmission. The proposed system shows how to deploy software components that are developed in consideration of the nature of 5G and MEC to achieve sub-1 second latency for 4K real-time video streaming. In the last of this paper, we analyze the most significant factor in the entire video transmission process to achieve the lowest possible latency.

        Exploratory Investigation for Improvement of the External Quality Assessment Scheme for Transfusion Medicine

        Background: This study summarizes a survey conducted to improve and expand the external quality assessment (EQA) scheme for transfusion in Korea. Methods: The EQA items provided by the College of American Pathologists, Royal College of Pathologists of Australasia, National External Quality Assessment Service, and German program were explored. A commercially operated EQA program was also included in the survey. Results: Items not included in the Korean EQA programs included extended antigen typing, electronic cross-matching, eluate survey, and fetal red cell detection. Granulocyte- and platelet-related testing was also not covered by the Korean program. There were issues regarding establishing the criteria for acceptable ranges and manufacturing of EQA materials. Conclusions: We successfully conducted a survey for the improvement and expansion of EQA for transfusion medicine. A pilot project for extended antigen typing is expected to be implemented in 2021, and ongoing expansion will continue in the following years.
