        Atypical Hepatocellular Carcinoma Mimicking Abscess on Multiphasic CT Images and Comparison between Imaging Finding and Gross Specimen

        Although multiphasic CT is an efficient technique for determination of hepatocellular carcinoma (HCC), occasionally, diagnosis of HCC using multiphasic CT can be difficult. Herein we present a case of HCC that was indicative of a typical abscess on CT imaging and compared the image findings with the gross specimen. The patient’s body temperature and laboratory results indicated that inflammation were in the normal range. This case emphasizes that not only imaging findings but also clinical presentation should be considered to achieve a correct diagnosis in clinical practice.

        Comparison of Standard and Specialized Readings in Routine Practice for the Assessment of Extraprostatic Extension of Prostate Cancer on MRI after Biopsy

        Purpose: To retrospectively determine whether specialized magnetic resonance imaging (MRI) reading performed by an experienced radiologist affected the successful assessment of extraprostatic extension (EPE) in the presence of biopsy-related hemorrhage after prostate biopsy. Materials and Methods: Two hundred consecutive patients with biopsy-proven prostate cancer underwent MRI. General radiologist and subspecialized radiologist readings were unpaired and reviewed in random order by a radiologist who was blinded to patients' clinical details and histopathologic data. The extent of hemorrhage was assessed on T1-weighted (T1W) MRI using a 1-4 scale, and the likelihood of EPE was assessed for each of the four categories. Histopathologic specimens served as the reference standard. The area under the curve (AUC) of the standard reading was compared to that of the specialized reading. Results: Post-biopsy hemorrhage was subjectively graded as ≥ 3 in 101 patients (50.5%) by standard reading, and in 100 patients (50.0%) by specialized reading. The standard and specialized readings disagreed for 40 (20.7%) of the patients (kappa [κ] = 0.35; 95% CI, 0.14-0.48). Of these, specialized reading was the correct interpretation for 21 patients (52.5%). The sensitivity (75% vs. 44%; P = 0.002) and area under the receiver operating characteristics (AUROC) (0.83 vs. 0.67; P = 0.008) of the specialized readings were significantly higher than those of the standard readings, while there was no significant difference in specificity (84% vs. 87%; P = 0.434). Conclusion: The reinterpretation of MRI by experienced radiologists significantly improves the diagnosis of EPE in prostate cancer in the presence of post-biopsy hemorrhage.

        K 대학병원 방사선사의 COVID-19(코로나19)에 대한 일반적 특성, 지식 및 행위와 상관성, 감염 예방 행위에 관한 연구

        본 연구의 목적은 K 대학병원에 근무하는 방사선사들의 COVID-19 에 대한 일반적 특성, 감염 관련 특성, 지식 및 행위와 상관성이 있는 요인, 감염 예방 행위에 영향을 미치는 인자를 파악 하는 데 있다. 기본 연구는 서술적 조사연구로 2020년 3월 25일부터 6월 30일까지 K 대학병원에 근무하는 방사선사 50명을 대상으로 자료를 수집하였다. 대상자의 설문 조사로 일반적 및 감염 관련 특성은 빈도, 백분율을 산출하였고, Pearson s 상관계수 분석하였으며, 감염 예방 행위에 영향을 미치는 요인은 다중 회귀분석을 이용하였다. 통계 검정의 유의 수준은 0.05로 설정하여 P-값이 0.05 미만이면 유의하다고 해석하였다. 연구결과에서 나타난 COVID-19 감염 예방 행위 영향을 미치는 인자는 코로나 지식이 1점 오를 때 감염 예방 행위가 1.7점 올랐고, 전파경로 인식 수준이 1점 올랐을 때 감염 예방 행위가 11.3점 올랐으며, 불안정도 1점 올랐을 때 감염 예방 행위가 4.2 점 올랐다. 표준 회귀계수를 보면 예방하는데 지식, 전파경로 인식, 불안 중 가장 큰 영향을 주는 인자는 코로나의 전파경로 인식이었다. 감염 예방 행위의 영향요인으로는 코로나 지식, 전파경로 인식과 불안이 감염 예방 가능성으로 나타나 코로나 유행 시 이 정보를 잘 이용하기를 기대한다. The purpose of this study is based on the convergence establishment of a coronavirus infection management system that can occur during clinical trials by grasping the knowledge of corona, infection possibility, infection prevention possibility, and implementation level of infection prevention behavior of radiologists working at K University Hospital. It is in providing data. This study was a descriptive research study, and data were collected from 50 radiologists working at K University Hospital from March 25 to June 30, 2020. The characteristics of the subjects and their knowledge of the COVID-19, the possibility of infection, the possibility of infection prevention, and the level of implementation of infection prevention actions were surveyed, and the collected data were analyzed with SPSS 25.0. The frequency and percentage were calculated for the general characteristics and infection-related characteristics of the subjects. The correlation between variables was analyzed by Pearson s correlation coefficient, and the factors influencing the progression of infection prevention behavior were analyzed by multiple regression analysis. Factors influencing COVID-19 infection prevention behavior shown in this study were 1.7 points for infection prevention behavior when corona knowledge increased by 1 point, and infection prevention activity increased by 11.3 points when the level of transmission pathway recognition rose 1 point. When the figure rose by 1 point, the infection prevention behavior increased by 4.2 points. When looking at the standard regression coefficient, preventive behavior is performed. Among knowledge, transmission path perception, and anxiety, the factor that has the greatest influence was the perception of the transmission path of COVID-19. As factors influencing the implementation of infection prevention actions, knowledge of COVID-19, awareness of transmission paths, and anxiety appear to be the potential of infection prevention, so in the event of a corona outbreak, information on infectious diseases and education on the possibility of infection prevention should be provided to promote the implementation of preventive action.

        Response Prediction after Neoadjuvant Chemotherapy for Colon Cancer Using CT Tumor Regression Grade: A Preliminary Study

        Purpose To investigate whether CT-based tumor regression grade (ctTRG) can be used to predict the response to neoadjuvant chemotherapy (NAC) in colon cancer. Materials and Methods A total of 53 patients were enrolled. Two radiologists independently assessed the ctTRG using the length, thickness, layer pattern, and luminal and extraluminal appearance of the tumor. Changes in tumor volume were also analyzed using the 3D Slicer software. We evaluated the association between pathologic TRG (pTRG) and ctTRG. Patients with Rödel’s TRG of 2, 3, or 4 were classified as responders. In terms of predicting responder and pathologic complete remission (pCR), receiver operating characteristic was compared between ctTRG and tumor volume change. Results There was a moderate correlation between ctTRG and pTRG (ρ = -0.540, p < 0.001), and the interobserver agreement was substantial (weighted к = 0.672). In the prediction of responder, there was no significant difference between ctTRG and volumetry (Az = 0.749, criterion: ctTRG ≤ 3 for ct- TRG, Az = 0.794, criterion: ≤ -27.1% for volume, p = 0.53). Moreover, there was no significant difference between the two methods in predicting pCR (p = 0.447). Conclusion ctTRG might predict the response to NAC in colon cancer. The diagnostic performance of ctTRG was comparable to that of CT volumetry.
