주상골 불유합에 대한 관절경적 골이식술 및 Kirschner-강선 고정술

Various surgical techniques, such as corticocancellous or cancellous bone graft and other vascularized bone grafting techniques have been developed to treat scaphoid nonunion. However, open grafting with dissection of wrist capsule and ligaments damages the joint and hence can lead to increased stiffness of the wrist and hand. Arthroscopic assisted bone grafting and percutaneous fixation have advantages such as minimal surgical trauma to the scaphoid blood supply and its ligament connection and provide a thorough wrist assessment, comprehensive approach for scaphoid nonunion and its sequelae in a minimally invasive manner. This article briefly discusses the characteristic anatomy of the wrist and scaphoid, and reviews the technique of arthroscopic bone grafting and percutaneous fixation for the treatment of scaphoid nonunion. 주상골 불유합을 치료하기 위해 지금까지 다양한 개방적 골이식 방법들이 소개되어왔다. 하지만 개방적 방법들은 수근관절막과 인대 손상을 유발해서 관절의 강직을 초래할 수도 있다. 관절경적 골이식술 및 경피적 고정술은 주상골의 혈액공급 및 주상골에 연결된 인대들에게 외과적 손상을 최소화하는 장점을 가진 방법이다. 또한 최소 침습으로 수근관절을 평가하고 주상골 불유합과 동반된 추가적인 손상을 진단 및 치료가 가능하다. 본 종설에서는 주상골 불유합 치료에서 관절경적 골이식술이 왜 필요한지 수근관절 및 주상골의 해부학적 측면에서 알아보고, 주상골 불유합 치료에 대한 관절경적 골이식술 및 경피적 Kirschner-강선 고정술에 대해 기술하도록 한다.
