        To understand the incident rate about the subtype of fibrocystic disease, we collected 116 breast cases requested from Jan. 1991 to June 1992, and reviewed, The results were as follows: 1. The fibrocystic disease was observed in 52.5%(61 of 116 cases). 2. The peak incidence was in the third decade and it did not reveal before the age of 20 and after the age of 50. 3. In fibrocystic disease, epithelial hyperplasia relative to breast cancer was observed in 33%(19 of 61 cases)and the epithelial hyperplasia was more significant in the young age group. 4. Among the 61 cases of fibrocystic disease, the most frequent type was fibrosis (88.5%), and followed by microcyst(32.8%), ductal hyperplasia(24.6%), lobular hyperplasia(11.5%), apocrine metaplasia(9.8%) and macrocyst and sclerosing adenosis(1.6%) in order.

        The soft tissue sarcoma is relatively rare tumor, comprising 1% of all the malignant tumors. Although precise pathogenic mechanisms had not been elucidated, the mutation of p53 has been the most strong candidate among cancer supressor genes. Most of the studies on the p53 mutation have been conducted in the carcinoma, rather than the sarcoma. The soft tissue sarcoma occurs any part of the body and the biologic behavior is unpredictable. The cellularity, necrosis and mitotic figures are prognostic factors of soft tissue sarcoma. Ki-67 normally expresses in all the cell cycles except Go phase. As it degrades rapidly after completion of mitosis, counting the cells expressing Ki-67 is a good prognostic factor. This study is conducted to see the role of p53 in the pathogenesis of soft tissue sarcoma and evaluate Ki-67 expression as the prognostic factor with 36 cases of malignant fibrous histiocytoma(MFH) and 26 cases of liposarcoma. The results were as follows: 1. According to the age group, the p53 was expressed each in 54.5%(6/11) and 68.6%(35/51) and the Ki-67 expression index was each in 52.7%±7.5 and 62.4±9.4 of below the age of 40 and above the age of 40 respectively. 2. According to the sex, the p53 was expressed each in 64.2%(27/42) and 70%(14/20), and the Ki-67 expression index was each in 48.9±10.5 and 66.2±11.8, of male and female respectively. 3. According to the histologic grade, the p53 was expressed each in 36.8%(7/19), 70.5%(12/17) and 84.6%(22/26), and the Ki-67 expression index was 31.8±9.3, 42.5±8.2 and 70.8±11.6, of grade 1,2 and 3 respectively. There was statistical difference between grade 1, and 3 for the expression of p53 (P<0.05). 4. According to the histologic types, the p53 was expressed each in 83.8%(30/36) and 42.3%(11/26), and the Ki-67 expression index was each 76.8±11.3 and 16.3±7.3. of MFH and liposarcoma respectively. There was statistical significant difference between the two types for the Ki-67 expression index (P<0.05). 5. According to the subtypes of MFH, the p53 was expressed each in 90%(27/30) and 60%(3/5), and the Ki-67 expression index was each 69.7±9.7 and 72.4±12.9, of common type and xanthogranulomatous type respectively. 6. According to the subtypes of liposarcoma, the p53 was expressed each in 33.3%(2/7), 25%(1/4) and 88.8%(8/9), and the Ki-67 expression index was each 8.6±5.3, 7.4±3.5 and 35.6±9.7, of myxoid type, round cell type and pleomorphic type respectively. The above results suggest that p53 mutation contributes in tumorigenesis less frequently with liposarcoma than with malignant fibrous histiocytoma. As the expression of p53 increased according to the high histologic grade and histologic types of poor prognosis, it would be useful prognostic factor. The Ki-67 expression correlates well with the degree of histologic grade and also could be useful prognostic factor in the soft tissue sarcoma.
