Steroid를 첨가한 Cardioplegic Solution의 心筋保護 效果에 관한 實驗的 硏究

The increasing use of cardioplegic solution for the reduction of ischemic tissue injury requires that all cardioplegic solution be carefully assessed for any protective or damaging properties. This study describes functional assessment of the efficiency of steriod in cardioplegic solution by using a Langendorff's perfusion model. Isolated rat hearts were subject to a 2 minute period of coronary infusion with the steriod mixed cold cardioplegic solution immediately before and also at the midpoint of a 60 minutes period of hypothermic(10±1℃) ischemic arrest. The result of this study were as follows: 1) Spontaneous heart, beat after ischemic arrest occured 14 second later Langendorff's reperfusion in the steriod mixed Young & GIK group and 16 second later in the control group.(Young & GIK without steriod) A good recovery state of spontaneous heart beat was shown in both groups. 2) The Percentage of recoveries of heart rate during the 30 minute after postischemic Langendorff's reperfusion was ; at first 5 minute 106.3±2.7%(p<0.05) in the steriod mix Young & GIK group. This percentage of recovery of the steriod mixed Yooung & GIK group was significantly greater than the control group during the first 5 minute course. 3) The percentage of recovery of coronary flow during the 30 minute after postischemic Langendorff's reperfusion was ; at first 5 minute 101±8.2% in the steriod mixed Young & GIK group. This percentage of recovery of the steriod mixed Young & GIK group was not significantly than the control group during the first 5 minute.

A vascular complication caused by lumbar disc surgery is not infrequent till recently after the report by Linton and White in 1945. Thereafter, many reports about the accidental injuries to the great vessels anterior to the lumbar area had been reported. In June, 1982, we experienced one case of arteriovenous fistula between right common lilac artery and inferior vena cava which was corrected surgically. The arteriovenous fistula caused by lumbar disc surgery and its review of the literature and presented.

Methylprednisolone을 첨가한 GIK용액의 심근보호효과에 관한 실험적 연구

Although corticosteroid have been shown to stabilize lysosomal membranes and prevent release of hydrolytic enzymes, the mechanism of membrane stabilization remains obscure. This study described functional assessment of efficiency of methylprednisolone in GIK solution by using a isolated Rat Heart Model. Isolated rat heart were subjected to a 2-minute period of coronary infusion with a cold GIK or methylprednisolone mixed cold GIK solution immediately before and also at the midpoint of a 60-minute period of hypothermic [$10{\pm}1^{\circ}C$] ischemic arrest. The result of this were as follow: 1.Spontaneous heart beat after ischemic arrest occurred 11 second later after Langendorffs reperfusion in the methylprednisolone mixed GIK group and 14 second later in the control group. 2.The percentage of recoveries of heart rate at 30 minute after postischemic working heart perfusion was 88.6\ulcorner.6% in the methylprednisolone mixed GIK group. This percentage of recovery was not significantly greater than the control group. 3.The percentage of heart function at 30 minute after postischemic working heart perfusion were; peak aortic pressure $90.8{\pm}4.5%$ coronary flow $87.5{\pm}1.45$ and aortic flow $74.9{\pm}11.8%$ in the methylprednisolone mixed GIK group. This percentage of recovery was significantly greater than the control group. [p<0.05]

For the quantitative analysis of inclusion using OES data, separating of noise and signal is needed. In the existing method assuming that signals come from a normal distribution, we use EM algorithm for Gaussian mixture to separate noise and signal. However, it is possible that the underlying distribution is a mixture of distributions other than Gaussian by characteristics of the data. In this paper, we assume that the distribution of OES data comes from a Gaussian-Gamma mixture and estimate the parameters of the mixture model using the proposed EM algorithm. Then, we calculate the mixing ratio of noise and inclusion using these parameters to separate noise and inclusion. We apply EM algorithms for mixture of Gaussians and Gaussian- Gamma to some real data and compare them with an appropriate measure.

For the quantitative analysis of inclusion using OES data, separating of noise and inclusion is needed. In previous methods assuming that noises come from a normal distribution, intensity levels beyond a specific threshold are determined as inclusions. However, it is not possible to classify inclusions in low intensity region using this method, even though every inclusion is an element of some chemical compound. In this paper, we assume that distribution of OES data is a Gaussian mixture and estimate the parameters of the mixture model using EM algorithm. Then, we calculate mixing ratio of noise and inclusion using these parameters to separate noise and inclusion.
