Purpose: To investigate cataract surgical experiences during ophthalmology residency in Korea. Methods: A survey regarding cataract surgical experiences during residency was sent to 4th year ophthalmologic residents (112) from April to September 2010. Results: Of the 112 individuals to whom the surveys were sent, 35 (31.3%) responded. The majority of the respondents (65.7%) performed their first cataract operation as the primary surgeon in their 4th year of residency. The majority of the respondents (42.9%) also thought that continuous curvilinear capsulorhexis (CCC) was the most difficult procedure followed by nucleus removal (31.4%). Fourteen respondents (40.0%) experienced loss of lens fragments into the vitreous cavity. Nine (25.7%) respondents had experienced performing ECCE (Extracapsular cataract extraction) during residency training. Also, 48.6% performed 1 to 5 cataract procedures during their entire residency training while 20.0% performed 6 to 10. Conclusions: The experiences in cataract surgery during residency were investigated for the first time through a questionnaire survey. The Korean ophthalmology residency program needs to provide more opportunities for residents to perform cataract surgery during their training period. J Korean Ophthalmol Soc 2011;52(4):429-433

Purpose: To compare the clinical outcome between Tecnis ZM900 and Tecnis ZA9003. Methods: We reviewed 20 eyes of 11 patients implanted with Tecnis ZM900 and 20 eyes of 20 patients implanted with Tecnis ZA9003. The clinical outcomes of these two intraocular lenses were evaluated one month after operation and consisted of distant, intermediate, and near visual acuity; depth of focus; contrast sensitivity; wavefront aberration; and patient satisfaction. Six months after the operation a reevaluation was performed for the group who had received the Tecnis ZM 900 implant. Results: Near and intermediate vision was better in the Tecnis ZM900 group. There were no statistical differences between the two groups with respect to distant vision and wavefront aberration. Contrast sensitivity was better in the Tecnis ZA9003 group and depth of focus was deeper in the Tecnis ZM900 group. There were no statically significant differences in the result between one month and six month after the operation. Conclusions: The Tecnis multifocal ZM900 IOL can be effective at improving patient satisfaction after cataract surgery as well as for correcting presbyopia. J Korean Ophthalmol Soc 2009;50(4):529-536

Latanoprost 점안 환자에서 발생한 단순포진결막염 1예

Purpose: To report a case of herpetic conjunctivitis in a glaucoma patient using latanoprost for IOP control. Case summary: A 65-year-old female patient using latanoprost for six months reported ocular pain on her right eye. Under slit-lamp examination, there were only some punctate epithelial erosions and mild peri-limbal conjunctival injection at 7 o’clock direction. The patient used levofloxacin (Cravit??, Santen, Osaka, Japan) antibiotic eyedrops and dexamethasone (Forus ointment??, Samil, Seoul, Korea) every 6 hours in a day for each. However, the patient reported the aggravation of the symptom and the conjunctival injection became more prominent than 4 days before. With fluorescein stain and yellow filter, a geographic pattern of ulcer lesion was noticed in the 7 o’clock peri-limbal bulbar conjunctiva. She was diagnosed with herpetic conjunctivitis. She was educated to stop latanoprost and received antiviral therapy. After two weeks, the patient showed good response to the treatment and there was no recurrence. Conclusions: Atypical herpetic conjunctivitis which present with geographic ulcerative lesion might happen in the glaucoma patients using topical latanoprost. Because of that, one should not omit to check conjunctiva in those patients and the fluorescein stain with yellow filter can help to increase sensitivity on the conjunctival lesion in the slit-lamp examination.

Purpose: To compare the osmolarity of various artificial tear products commercially available in the Republic of Korea. Methods: The osmolarity of the artificial tears was analyzed using TearLab?? and Multi-Osmette™. We divided the artificial tear products into five groups, including hypo-osmolar 0.18% sodium hyaluronate (HA), iso-osmolar 0.1%, 0.15%, 0.3% HA, and 0.5% carboxymethylcellulose sodium (CMC), and measured three products per group. Results: Among the 12 products of iso-osmolar artificial tears, there were seven products (58.3%) < 300 mOsm/L, and among the three products of hypo-osmolar artificial tears, there were two products (66.6%) < 150 mOsm/L. A significant difference (p < 0.05) was observed between the iso-osmolar 0.3% HA group and the hypo-osmolar 0.18% HA group. In the iso-osmolar 0.1% HA and 0.15% HA, the difference between the products in each group was approximately 12 mOsm/L, and the CMC group had the smallest deviation among products, which was close to 300 mOsm/L. There was a significant difference of 305.57 ± 6.38 mOsm/L in the iso-osmolar 0.15% HA group, 293.44 ± 8.71 mOsm/L in the 0.1% HA group, and 285.67 ± 3.67 mOsm/L in the 0.3% HA group (p < 0.001). There was no significant difference in osmolarity between the iso-osmolar products containing HA and CMC (p = 0.117). Conclusions: There was a difference in osmolarity between the published and actual measured values of commercial artificial tears, and there was a difference in osmolality depending on the content of HA and the manufacturer.
