Background and Objectives:Nasal polyps are the most comon mas lesion of the nasal cavity and it occurs on both infe-ctious and noninfectious basis, and especialy because genetic etiology is suspected in the development of nasal polyp. Tumor necrosis factor-α(TNF-α) is a pivotal cytokine in inflamatory and infectious diseases, and increased tissue TNF-α levels have been observed in the inflammatory type of nasal polyp. It has been known that several TNF-α gene polymorphisms in a promoter region are related to TNF-α production. Among them, the G to A substitution at the position 308 (TNF-α 308A) and at the position 238 (TNF-α 238A) in the TNF-α promoter are associated with the high or low production of TNF-αwe investigated the association between with TNF-α promoter polymorphism and nasal polyps. Subjects and Methods:This study included 104 patients with polyp asociated chronic sinusitis and 104 healthy controls. TNF-α promoter genotype was screned by PCR-restriction fragment length polymorphism (PCR-RFLP). Results:In the nasal polyp associated chronic sinusitis, the frequency of TNF-α 308A was higher than those of healthy controls (OR, 1.739;95% CI, 0.653- 4.63 ). But there was no statistical significance between distribution of TNF-α gene polymorphism and nasal polyps. Frequency of TNF-α 238A was higher in nasal polyp than in healthy controls, but no significant differences were found. We concluded that TNF-α polymorphism did not show genetic predisposition with regard to nasal polyp in Korean patients.

Background and Objectives:Nitric oxide (NO) is an endogenous fre radical gas mediator, synthesized by a family of nitric oxide synthases (NOS). Established properties of NO of potential relevance to the formation of nasal polyps include vasodilatation, direct regulation of eosinophil and neutrophil function, and potentiation of histamine-induced plasma exuda-tion. Steroids are currently the most potent medication available for the treatment of nasal polyposis, but the exact mechani-and long-term topical steroid in nasal polyps. Materials and Method:Steroid-untreated nasal polyp patients (n=10), oral steroid-treated nasal polyp patients (n=10, prednisolone 30 mg per day for 7days) and topical steroid-treated nasal polyp patients (n=10, Fluticasone 100 μg per day for more than 1 month) underwent nasal endoscopy and biopsy of the polyps. The protein expresions of NOS in nasal polyp tissues were evaluated by immunohistochemistry. Results:chemical studies revealed that expression levels of proteins produced by three NOS isoenzymes were significantly decreased in steroid-treated nasal polyps when compared with steroid-untreated nasal polyps, and there was no diference in the effects betwen short-term systemic and long-term topical steroid treatment on nasal polyps. Conclusion:These results show that the expresions of constitutive NOS as well as inducible NOS in nasal polyp tissue were suppresed by steroid therapy. There was no diference in the NOS isoenzymes expression between short-term systemic and long-term topical steroid-treated polyps. (Korean J Otolaryngol 2002;45:765-70)
