Cutaneous leiomyosarcoma is a rare soft tissue tumor that accounts for 2∼3% of all soft tissue sarcomas. Cutaneous leiomyosarcoma can be subdivided into dermal, subcutaneous, and secondary leiomyosarcomas. This subdivision is important because of the different prognosis according to the types. The dermal type of cutaneous leiomyosarcomas are small and grow in the dermis. They rarely metasize, and the prognosis for survival after excision is favorable. We describe a case of dermal type cutaneous leiomyosarcoma on the extensor surface of the right thigh in a 35-year-old man. (Korean J Dermatol 2007;45(9):938∼941)

Korean FDA announced the revised enforcement rules of cosmetic act on January 12, 2017 and they revealed the rules will become effective on May 30, 2017. According to the revised rules, the number of cosmetics is extended to 10 species of cosmetics; from previous 3 types of whitening/improvement of wrinkles/hair removal to 10 types containing hair dye/hair removal/improvement of hair loss/improvement of acne skin/improvement of atopic skin/improvement of red lines by stretch marks. Given that it is not yet finalized, we need to have the time to study the basics of the hair-related product, as it is necessary to have a basic knowledge of the product as a dermatologist. 1. Cosmetic products that has the ability to change colors of hairs. 2. Cosmetic products that has the ability to remove hairs of bodies. 3. Cosmetic products that help relieving hair loss symptoms. Although dermatologists are experts in managing scalp and hair disease, the esthetic of some cosmetic therapies still remain elusive. Knowing thir mode of action, safeness and ingredients will enable the physician to better assess different problems secondary to cosmetic treatments.

Cutaneous pseudolymphoma has been defined as a benign lymphocytic infiltrate of the skin that resembles cutaneous lymphoma both clinically and histologically. There are many known causes of skin pseudolymphoma, but a definite cause cannot frequently be identified. We report here on a 37-year-old female with pseudolymphoma on the left ear lobe after having her ear pierced. When we performed a biopsy, an earring was found by chance. Unlike most of the reported cases of pseudolymphoma after ear piercing, this patient did not show a delayed-type hypersensitivity reaction to gold or nickel. (Korean J Dermatol 2008;46(10):1424∼1426)
무모생쥐를 이용한 Disk 형태(DTS) 미세침롤러의 피부안정성에 관한 연구

Background: The transdermal drug delivery system has been studied for several decades as an alternative tool. Even though it has many advantages such as stability of drug to the destination, safety to the liver, there are some limitations in crossing the stratum corneum. Microneedle can perforate the skin directly, which make it possible for drugs to penetrate into the subcutaneous tissue. Objective: The purpose of this study to evaluate the stability of Diskmicroneedle Therapy System (DTS) and directly compare them with conventional Microneedle Therapy System (MTS(R), Clinical Resolution Lab, California, USA). Method: We used 3 sizes of Manual and electric Disk-microneedle Therapy System (DTS; 0.25 mm, 1.0 mm and 1.5 mm) to perforate the skin. We performed a comparative study of 0.5 mm manual DTS and Microneedle Therapy System (MTS(R)). To evaluate the stability, visual inspection and biopsy were conducted. Result: Manual and electric DTS of all 3 sizes did not induce significant abnormalities of the skin on visual inspection and biopsy. DTS and MTS produced similar results on the stability for the skin. Conclusion: Similar to the preexisting MTS, DTS is a safe transdermal delivery system, and the size should be chosen carefully. Symposium 1-3 (SYP 1-3) : Photoprotection in acne

Proper skin care is considered to be an important component of the total management plan for patients with acne vulgaris. Many acne patients may not consider it necessary to use a photoprotection as part of their skin care regimen, but it is an essential part of restoring a balanced barrier and long-term skin health. Many acne therapies thin the skin and predispose to ultravilolet (UV) damage, and UV damage itself has been shown to generated free radical formation, which has been implicated in acne flares. Furthermore, Patients with acne are frequently prescribed topical retionoids, oral tetracyclines, BPO-containing formulations, etc, that necessitate photoprotection. Sebaceous gland hyperplasia and increased sebum secretion after irradiation of UV-B has been widely accepted. Hyperplasia of the sebaceous gland after irradiation of UV-B was proved by biopsy in a hairless murine model. Proliferation of cultured sebocytes in hamsters was increased by 40% after 4 days of UV-B irradiation, and sebum production was increased by 410% on day 7. Sebum secretion and production of cytokines (IL-1a, IL-6, IL-1 receptor antagonist, IL-10) from open comedones of acne patients were increased in the first several days both after irradiation of UV-A and UV-B, but subsequently returned to normal values. On the other hand, acne patients have sometimes irritation or worsening by the sunscreen itself. Therefore, it is crucial to counsel acne patients regarding sun protection. Here I would like to discuss about the necessity of photoprotection, as well as our concern about the choice of right sunscreen for acne patients.

Bullous pemphigoid (BP) is a chronic autoimmune subepidermal blistering disease that usually occurs in the elderly. Two types of clinical variants exist. The generalized form typically involves the lower abdomen, groin, thighs and inner arms. The localized forms of BP include (1) mucous membrane pemphigoid or cicatrical pemphigoid, (2) localized scarring pemphigoid or Brunsting-Perry pemphigoid, and (3) localized nonscarring pemphigoid. We report here on a case of localized nonscarring pemphigoid on the face of a 71-years-old man. (Korean J Dermatol 2008; 46(11): 1526∼1529)
