제II형 글루타르산혈증은 발생 시기와 정도에 따라서 질병의 경과를 달리한다. 지발형의 경우 평상시에는 증상이 없고, 검사 소견도 정상 소견을 보이나 대사가 증가되는 hypercatabolic status에서는 저혈당, 대사성 산혈증, 고암모니아혈증 등이 나타나며, 다양한 신경학적인 증상들이 나타날 수 있다. 따라서 신경학적인 증상들이 있는 경우 기본적인 검사들을 시행해 보고, 위와 같은 소견들이 보이는 경우에는 소변의 유기산 분석을 통한 진단이 필요하다. 정맥내로 포도당이 투여되면 유기산 분석의 양상이 매우 빠른 속도로 정상화될 수 있으므로 정맥내로 포도당을 투여하기 전에 소변 검체를 채취하여 냉동 보관하는 것은 정확하고 빠른 진단을 위하여 필수적이라고 할 수 있다. Glutaric acidemia (GA) type II is a very rare inherited disorder that have no accruate figure on its icidende. People with Glutaric acidemia type II have an enzyme that does not work properly. Two specific enzymes are associated with Glutaric acidemia type II:1. Electron transfer flavoprotein (ETF), 2. ETF-ubiquinone oxidoreductase (ETF-QO). Both of these enzymes have similar functions in the body, and children with Glutaric acidemia type II may lack one or the other of these enzymes. They play an important role in breaking down fats and proteins, and help the body to produce energy. GA II clinically manifested as (1) neonatal onset with congenital anomalies (2) neonatal onset without anomalies, and (3) mild and/or later onset. The first two groups are sometimes said to have multiple acyl CoA dehydrogenation deficiency-severe and the third to have multiple acyl CoA dehydrogenation deficiency-mild. The course and age at presentation of later-onset glutaric acidemia type II is extremely variable, therefore it is difficult to diagnosis. We experienced one case of late onset form glutaric acidemia type II with afebrile status epilepticus-like convulsion.

소아 경련성질환의 Valproate 치료에 의한 혈소판감소증의 유병률 및 위험인자

목 적: valproate 치료에 의한 혈소판감소증의 유병률과 혈소판 수의 변화에 영향을 줄 수 있는 위험인자에 대해서 알아보고자 본 연구를 시행했다. 방 법: 2000년 1월 1일부터 2005년 12월 31일까지 경련성 질환의 치료에 valproate를 단독 사용한 환아들 중 1년 이상 추적이 가능했던 54례를 대상으로, 혈소판감소증의 유병률과 연령과 성별, valporate의 혈중농도, 치료기간 등과 혈소판 수와의 관계를 분석하였다. 결 과: 소아에서 valproate 치료에 의한 혈소판 감소증은 11.1%였다. 치료전 혈소판 수는 332,240±97,820/μL였으며, 3개월 270,740±100,400/μL, 6개월 273,640±86,660/μL, 12개월 300,850±108,850/μL로 3개월 이내 현저한 혈소판 수의 감소를 보였다(P>0.05). 성별, 나이 그리고 혈중농도에 따른 혈소판 수의 차이는 없었다. 결 론: valproate 치료 후 혈소판 감소는 대부분 치료시작 3개월 이내 발생하며 치료를 하지 않아도 정상 혈소판 수로 회복되나. 일부에서는 중증의 혈소판 감소로 출혈성 경향을 보일 수 있어 주기적인 검사가 필요할 것으로 생각된다. Purpose : This study was undertaken to investigate the prevalence and the risk factors of thrombocytopenia in children taking valproate for convulsive disorders. Methods : From Jan. 2000 to Dec. 2005, we reviewed the medical recordes of 54 children with convulsive disorders taking valproate over a period of 12 months. Then, we estimated the prevalence of thrombocytopenia, and analyzed the relationship between changes in platelet counts and other factors such as age, sex, serum valproate levels and the duration of valproate therapy. Results : Six(11.1%) children taking valproate had at least one episode of thrombocytopenia. The platelet counts were significantly lowered at 3 months after valproate therapy and recovered after 6 months(P<0.05). The relationship between the platelet counts and other factors such as blood valproate levels, sex and age was not observed in our study. Conclusion : Thrombocytopenia can appear after a few months of therapy and regular monitorings of platelet counts in children taking valproate may be needed to prevent potential bleeding diathesis.
