      • 고속스핀에코기법을 이용한 MRI검사에서 ETL 변화에 따른 영상 blurring의 평가

        The purpose of this study is to evaluate image blurring according to variation of the ETL and propose the clinically appropriate ETL range. SIEMENS MAGNETOM Skyra 3.0T and 20 channel head coil were used for the study. MRI phantom was kept the lines horizontally to three direction(X,Y,Z) of the coil and T1, T2 weighted images that used the fast spin echo technique acquired. The ETL with increase of 10 was applied from 10 to 80. In addition, the ETL with increase of 1 was applied in the interval statistically significant differences occurred. And T1, T2 weighted images that used the conventional spin echo technique acquired to compare image blurring of the images that used the fast spin echo technique. The slope of lattice in the images was measured using Image J 1.47v program to evaluate image blurring. And image blurring was determined by the degree of the slope. The statistical significance of both techniques was evaluated by the Kruskal-Wallis test of the SPSS 17.0v. And the correlation of the ETL and image blurring was evaluated quantitatively by regression analysis. The slope of the T1, T2 weighted images that used fast spin echo technique decreased as contrasted with conventional spin echo technique. In the result of the Kruskal-Wallis test, the T1, T2 weighted images that used fast spin echo technique made a significant difference with conventional spin echo technique. Particularly, in the Tomhane' T2 test, the T1, T2 weighted images made a significant difference from ETL 22 and 31 respectively. In the result of the regression analysis, the R-squared of the T1, T2 weighted images are 0.762 and 0.793. It is difficult to apply the long ETL in the T1 weighted image caused by the short TR and multi-slices study. Therefore, clinical impact according to variation of the ETL is very slight in the T1 weighted images. But the application of the proper ETL is demanded in T2 weighted images using the fast spin echo technique in order to prevent image blurring.

        중등도 무지외반증에서 시행한 단축 스카프 절골술

        Purpose: The aim of this study was to evaluate the radiographic and clinical results of short scarf osteotomy that has minimized longitudinal cut for moderate hallux valgus. Materials and Methods: Total 12 patients (12 feet) were reviewed by medical records and radiographs. All patients were female and the mean age at the time of operation was 41.5 years. The mean followup time was 21.2 months. We modified original scarf osteotomy by shortening the longitudinal cut to 15~20 mm in length. Additionally, Akin osteotomy of the first proximal phalanx was done in 7 feet and Weil osteotomy of the second metatarsal was done in 4 feet. First-second intermetatarsal and hallux valgus angles were analyzed radiographically before and after the operation. And the clinical result was assessed by AOFAS (American Orthopaedic Foot and Ankle Society) hallux score. Results: First-second intermetatarsal and hallux valgus angles were reduced from the mean preoperative values of $14.6^{\circ}$ and $32.8^{\circ}$ to $6.5^{\circ}$ and $11.2^{\circ}$, respectively. The mean AOFAS hallux score was increased from 52.4 points preoperatively to 88.2 points at followup. Three complications were found: metatarsal fracture during the operation, painful scar around second metatarsal head after Weil osteotomy and postoperative neuralgia. There was no transfer metatarsalgia or recurrence of hallux valgus during followup. Conclusion: Short scarf osteotomy would be an effective surgical procedure for moderate hallux valgus with the benefits of minimized soft tissue dissection and stable fixation.

        다공성 수산화인회석 혼합 골시멘트의 골내성장 효과

        The purpose of this study is to investigate the bone ingrowth effect of porous hydroxyapatite(HA) impregnated bone cement(polymethylmethacrylate, PMMA) through the histologic and biomechanical analysis. For this study, HA and PMMA were mixed in weight ratio 0:100, 10:90, 30:70, to make 0%, 10%, 30% mixture cylindrical rod shaped sample, sized about 4mm x 10mm. The halves of each sample as control were decalcified to make a comparative histologic and biomechanical analyses of bone ingrowth effect of HA impregnated PMMA. The histologic and biomechanical analyses in vitro and vivo were undertaken to evaluate the bone ingrowth effect of HA impregnated PMMA in several aspects to make a comparative analysis of the interfacial shear strength and a histologic analysis of HA impregnated PMMA in vivo. The comparative analysis of interfacial shear strength using hydraulically controlled machine(INSTRON) in vitro and histologic examination of HA impregnated PMMA in vivo were undertaken using each HA exposed samples(by grinding off and removing the surface coated PMMA) and those processed by surface decalcification, which were harvested from both sides of distal femurs of rabbit at 6 weeks after implantation, respectively. The following results were obtained; 1) In histologic observation using the hard tissue sectioning method with Villaneuva bone stain, there was peri-implant new bone formation in HA impregnated PMMA and more remakable in 30% samples, but an osseointegration into the HA was observed only in 30% sample. And in 10Po and 30% samples decalcifying off the surface HA, peri-implant new bone formation was observed also and more remakable in 30%. But there was no definite difference of bone ingrowth related periimplant new bone formation after and before decalcification about each 10% and 30% samples.

        대퇴골 전자간부 불안정 골절에 대한 골시멘트 보강 압박고나사의 내고정 치료

        목적: 골시멘트를 대퇴골두 내로 주입하여 보강한 후 압박고나사를 내고정하는 방법으로 치험한 대퇴골 전자간부 불안정 골절의 임상적 및 방사선적 결과 분석을 시행하였다. 대상 및 방법: 1996년 5월부터 2002년 5월까지 골시멘트 보강 압박고나사 내고정 방법으로 치료하고 1년 이상 추시 가능 하였던 전자간부 불안정 골절 30예를 대상으로 하였고, 평균 연령은 74.8세(51-89세), 추시 기간은 평균 16.8개월(12-36개월)이었다. 방사선적으로 골다공증의 정도, 내고정 후 경간각, 압박고나사의 골두 내 위치, 완전 유합시 압박고나사의 활강 정도(Doppelt’s method)와 하지길이의 단축, 골시멘트의 충진 정도, 방사선적 유합 시기 등을 평가하였고, 임상적으로는 보행 능력의 변화(Koval의 분류)를 평가하였다. 결과: 방사선적으로, 골절은 평균 7.6° 외반위 정복되었고 내반위로 정복된 예는 없었으며, 압박고나사의 골두 내 위치는 후하방 13예, 중앙 8예 등의 순으로 많았고, 골유합 시기는 평균 16.3주였다. 추시 기간 동안 총 30예 중 10° 이상 내반 변형이 5예, 15㎜ 이상 압박고나사 활강이 7예, 지연유합이 2예였으나, 골두천공이나 금속고정 실패는 없었고, 최종적으는 모두 만족스런 골유합을 얻을 수 있었다. 임상적으로도 Koval에 의한 보행 능력 평가상, 대부분의 예에서 만족할 만한 결과를 얻었다. 결론: 골시멘트 보강 압박고나사 내고정 방법은 대퇴골 전자간부 불안정 골절의 수술적 치료 후 발생 가능한 골두 천공, 정복 소설 등을 줄이고, 수술 후 조기 거동을 가능하게 하여 오랜 침상 가료로 인한 합병증을 감소시킬 수 있는 좋은 방법 이라고 사료된다. Purpose: The aim of this study was to evaluate the clinical and radiological results of a PMMA augment¬ed compression hip screw for the management of unstable intertrochanteric fractures of the femur in senile patients. Materials and Methods: From May 1996 to May 2002, 30 patients (mean age 74.8 years, ranging from 51 to 89) with an unstable intertrochanteric fracture of the femur, were treated with a PMMA augmented compression hip screw. The average follow-up period was 16.8 months (12 to 36). Radiologically, the degrees of osteoporosis, the neck-shaft angle after the reduction, the position of lag screw in the femoral head, the degrees of sliding of the lag screw and the shortening of the limb when union had been achieved, the extent of the insertion of the PMMA cement, and the timing of bone union were analyzed. In addition, the changes in the ambulatory capacity were evaluated by the functional class of Koval. Results: Radiologically, 5 cases of a varus deformity over 10° and 7 cases of lag screw sliding more than 15 mm were found in the 30 cases. However, there was no cutting out of the screw in femoral head nor was there fixation failure of the plate. In addition, union was finally achieved in all cases, although there were two cases of a delayed union. Clinically, satisfactory results were obtained in most of the cases according to the functional class of Koval. Conclusion: For senile patients with an unstable intertrochanteric fracture of the femur, PMMA augmented compression hip screw appears to be one of the more useful methods allowing the early ambulation and reducing the number of complications by the long immobilization.

      • 알루미늄 합금판재의 접합을 위한 접착제의 적용연구

        This study was carried out to investigate the possibility of applying adhesive to autobody manufacturing. Since the strength of joints is affected by pressure. temperature and surface conditions,<br/> the effects of those factors were investigated under various conditions. Adhesive joints specimens were exposed to outdoor for one<br/> year to examine the environmental effects. It was found that the joint strength was not deteriorated even under the tenstion loads. Adhesive joining is comparable to laser welding in terms of shear tension force and higher than resistance spot welding and mechanical clinching.<br/>

        This study was carried out to evaluate the zinc phosphatibility of steel, Zn-Ni coated steel and aluminum alloy<br/> sheets. Paint adhesion tests shows that the zinc phosphating conversion coatings formed on steel, Zn-Ni coating and aluminum surface performed excellent and satisfied the requirements, except<br/> one type of aluminum sheets. The inferiority of this aluminum sheet was found to be caused by a thick MgO oxide layer and insufficient degreasing of the surface. It should be noted that scratches on the surface inhibited the zinc phosphating of aluminum sheets.<br/> Key Word: Zinc Phosphating, Aluminum, Steel, Zn-Ni Coated Steel, Paint Adhesion, Oxide Film, Scratch.<br/>

      • 지방자치단체의 효율적 예산관리에 관한 연구

        최근 국내의 환경변화는 경쟁의 구심점이 중앙에서 지방으로 이동됨에 따라, 지방자치단체의 환경변화에 대한 대응능력이 매우 중요시되고 있다. 이러한 중요성은 점차적으로 커지고 있으며, 이에 따라 지방자치단체는 계획적이고 체계적이고 합리적인 재정 운영을 통해 지역발전의 실현가능성을 높이기 위해 노력을 하고 있다. 지방의 발전은 지방자치단체의 예산제도에 의해 성공적으로 이루어지게 되며 이러한 운영과정에서 가장 핵심적인 요소는 예산의 편성과 및 집행 및 결산을 통한 자원의 효율적 사용이다. 본 연구는 지방자치단체의 예산편성 및 운영관리에 대한 현상을 분석하고 이를 개선 할 수 있는 방안을 모색하여 지방자치단체의 예산이 효율적으로 관리될 수 있도록 하는데 그 목적이 있다. 현행 지방 자치단체에서 채택하고 있는 예산제도는 1 회계 연도 중심의 품목별 예산 제도이다. 품목별 예산제도는 기본적으로 예산을 지출대상의 기능ㆍ조직ㆍ성질별로 통일적이고 체계적인 관리를 위해 예산과목을 구분한 것으로서 수입과 지출을 정부거래의 본래적 목적 및 성격에 따라 체계적으로 분류한 것이 아니기 때문에 정부의 재정 상태와 운영성과 등을 종합적으로 설명하는 데는 많은 한계점을 지니고 있다. 또한 예산과 회계처리과정에서도 예산과목별로는 결산서를 통해 예산과 실적의 대비가 가능하지만 사업별로 예산과 실적을 비교할 수 없으며 달성된 성과를 파악하기 곤란한 점이 있다. 따라서 주민이 원하는 사업이 쉽게 납득할 수 있도록 파악되고 주민의 편익을 극대화하기 위한 방안으로 예산이 관리되어야 할 것이다. 1 회계연도 중심의 예산제도는 수 년도에 걸쳐 수행되는 자본투자 사업에 대해 사업기간별 추진실적과 자원사용내역을 적실성 있게 보여주지 못하고 있다. 그리고 현재의 1년 단위의 예산회계제도는 예산회계연년도의 종료와 개시를 전후하여 1년마다 사업이 단절되어 사업추진의 효율성이 떨어질 뿐만 아니라 사업추진의 효율성이 저해되며, 회계연도에 쫓겨 사업의 부실화를 초래한다. 그리고 회계연도 개시 약 1개월 동안 행정 업무가 정체되며, 회계연도내의 예산지출의 완료를 위하여 불필요한 예산지출 관행이 초래하는 문제점을 가지고 있다. 이러한 문제점 때문에 지방자치단체의 예산과리의 효율성을 높이기 위해서는 크게 두 가지의 개선방향이 요구된다. 첫째는, 현재 지방자치단체에서 사용하고 있는 품목별 예산제도를 사업별로 이루어 질 수 있는 사업별 예산제도로 전환하여야 한다. 둘째는, 현재 지방자치단체에서 사용하고 있는 1년 단위의 예산회계년도를 다년도 예산제도 (multi-year budgeting) 등을 통해 예산운영의 유연성을 높이는 것이 필요하다. Recently the importance of the local governments' activities for the use of their resources are increasing. Accordingly, local governments increases their efforts to the systematic and rational operation of the public finance. The local's growth primary depends on the efficient use of the resources and the budgetary system in local governments. The key factor for the budgetary system lies on the formulation, execution and closing accounts of the budgeting. The purpose of this study is to examine the formulation and operation of budgeting in local governments, and suggests the betterments of the efficient managements for the budgeting. Current budgetary system in local governments uses one yeat budgeting based on items. The budgetary system primary based on items classifies the expenditures on item by item, such as material, labor, and overhead. However this budgetary system lacks explanations for the overall financial positions and operational results in local governments, because this system does not classifies the revenues and expenditures on the purposes and functions. Furthermore, this system cannot compare the planned budgets and the accomplished results of the projects and business. Current one-year budgetary system can not provide the information for the propositional accomplishments of the multi year projects. Furthermore the current system often reduces the efficiency of the business in the beginning and end of the budget year due to cut off time of the budget year. Another limitation of this system is the use of the unnecessary expenditures due to the budgetary slack. Considering these problems in the budgetary system in local government, this paper suggests two improvement approaches for the budgetary system in local government to enhance the effectiveness and the efficiency of the public sector. One approach in local governments' budget ing system is to adopt programming budget from budging bases on items. The other approach in local governments' budgeting system is using a multi-year budgeting to enhance flexibility of the budget management.
