        Objective; Secondary hypertension is known to account for about 5-10% of hypertension. Recently, a number of soldiers complaining of high blood pressure have been found in military health checkups. This study aimed to report the prevalence of secondary hypertension among young soldiers in a single military hospital. Methods; We obtained clinical information of young hypertensive soldiers under 40 years of age who visited the Armed Forces Chuncheon Hospital from 2016 to 2018. Among them, we retrospectively analyzed the medical records of soldiers who underwent secondary hypertension screening due to high blood pressure. Results; Between January 2016 and December 2018, a total of 699 hypertensive soldiers under 40 years of age visited our hospital. Among them, 299(100% men) soldiers underwent secondary hypertension screening. The mean age was 21.6 ± 3.1 years old. The mean blood pressure levels were 154.5 / 91.5 mmHg with 124 soldiers(41.5%) of stage 2 hypertension, 157 soldiers(52.5%) of stage 1 hypertension, and 18 soldiers(6%) of prehypertension. Secondary hypertension was diagnosed in 2 soldiers among 299 cases. The cause of secondary hypertension was identified as hyperthyroidism in 2 soldiers. Conclusions; The prevalence of secondary hypertension in young soldiers under 40 years of age with hypertension was 0.3%.

        변형된 비속어 탐지를 위한 토큰 기반의 분류 및 데이터셋

        기존의 비속어 탐지 방법들은 의도적으로 변형된 비속어를 식별하는 데 한계가 있다. 이 논문에서는 자연어 처리의 한 분야인 개체명 인식에 기반한 새로운 방법을 소개한다. 우리는 시퀀스 레이블링을 이용한 비속어 탐지 기법을 개발하고, 이를 위해 한국어 악성 댓글 중 일부 비속어를 레이블링하여 직접 데이터셋을 구축하여 실험을 수행하였다. 또한 모델의 성능을 향상시키기 위하여 거대 언어 모델중 하나인 ChatGPT를 활용해 한국어 혐오발언 데이터셋의 일부를 레이블링을 하는 방식으로 데이터셋을 증강하여 학습을 진행하였고, 이 과정에서 거대 언어 모델이 생성한 데이터셋을 인간이 필터링 하는 것만으로도 성능을 향상시킬 수 있음을 확인하였다. 이를 통해 데이터셋 증강 과정에는 여전히 인간의 관리감독이 필요함을 제시하였다. Traditional profanity detection methods have limitations in identifying intentionally altered profanities. This paper introduces a new method based on Named Entity Recognition, a subfield of Natural Language Processing. We developed a profanity detection technique using sequence labeling, for which we constructed a dataset by labeling some profanities in Korean malicious comments and conducted experiments. Additionally, to enhance the model's performance, we augmented the dataset by labeling parts of a Korean hate speech dataset using one of the large language models, ChatGPT, and conducted training. During this process, we confirmed that filtering the dataset created by the large language model by humans alone could improve performance. This suggests that human oversight is still necessary in the dataset augmentation process.

      • 징병신검과 입영신검과의 연관성 및 추이분석

        Objectives: The purpose of this study is to analyze the shift of men who are deferred to be recruited at physical examination held in the conscription agency and in the enlisted unit. Methods: The subjects of study are 542,828 men who have all received physical examination after being enlisted in army between January 2013 and June 2015. Data received from the ROK Army Personnel Command and Miliary Manpower Administration was utilized for the study. Results: The main results of this study was, firstly, men who were deferred from recruitment among total men enlisted were 2.1%. Secondly, the deferred rate based on time period between the point of physical examination held at the Military Manpower Administration and physical examination held at the enlisted unit showed increasing trend as time elapsed. Particularly, the rate increased drastically after 6 years. Thirdly, departments for treatment mostly visited by deferred men were psychiatry, orthopedics, neurological surgery and dentistry in order. Conclusions: he result of this study strongly suggests the need to transform the current physical examination policy which divided the examination into conscription physical examination and enlistment physical examination and such policy can arose confusion in those who are subject to be enlisted and negatively affect trust against military. Therefore, it is in dire need to abolish current policy of physical examination for enlisted soldiers or to replace it to provisional examination, and also require research and decisions of policies regarding plan to hold physical examination at the Military Manpower Administration before enlisting into army.

      • 한국군 코로나19 감염자의 초기 역학적 특성

        Introduction; South Korea is one of the exemplary countries that have successfully responded to COVID-19 pandemic with the South Korean military having only a small number of confirmed patients. This study aimed to provide basic data for establishing a strategy in response to COVID-19 in the future by analyzing the epidemiological and clinical characteristics of patients in the Republic of Korea Armed Forces military. Method; The materials were collected through electronic medical records and telephone interviews, while descriptive analysis was performed for the statistical analysis of clinical and epidemiological characteristics. Result; From February 20 to September 2, 2020, 110 patients were confirmed cases of SARS-CoV-2 infection in the ROK military. The number of confirmed cases due to cluster infection was 63.6%, of which 62.9% were enlisted. The prevalence of COVID-19 in the ROK military was 17.7 per 100,000 persons. The median age was 22 years, and 94.5% of patients were men. The primary infection rate was 59.4%, followed by 34.2%, and 5.4%. The common symptoms were fever and cough during the entire hospitalization period. Moreover, radiological diagnosis of pneumonia was confirmed in 17.3% of patients. The incubation period was five days, and there was no deceased case among the confirmed cases; hence, the case fatality rate was 0%. Conclusion; The timing and region of the outbreak in patients in the military was affected by the pattern and status of confirmed cases in the entire country. Enlisted were more vulnerable and affected mainly by cluster infection. The military response to COVID-19 has been effective so far, while the military can prevent the spread of COVID-19 by implementing a stronger quarantine system to preserve combat power.
