      • The positive culture rate and antimicrobial susceptibilities of Mycoplasma hominis and Ureaplasma urealyticum

        Objective: The Objective of this study was to assess the positive culture rate and antimicrobial susceptibilities of Mycoplasma hominis (MH) and Ureaplasma urealyticum (UU) in general population and pregnant women. Methods: We reviewed a retrospective medical records of patients who have undergone culture test and antimicrobial susceptibilities at our center from April 2018 through January 2017. The study included all patients with positive culture for MH, UU or both. Results: The condition was assessed in 200 patients. All of them, 34 (17%) were pregnant women and 166 (83%) were non-pregnant women. From 105 (52.5%) out of total 200 had a positive culture as follows: MH 46 (23%), UU 95 (47.5%) and both 36 (18%). Susceptibilities of MH to doxycycline, erythromycin, ciprofloxacin, and azithromycin were 97.8%, 6.5%, 17.4%, and 8.7%, respectively. Susceptibilities of UU to doxycycline, erythromycin, ciprofloxacin, and azithromycin were 95.8%, 55.8%, 7.4%, and 56.8%, respectively. Susceptibilities of both MH and UU to doxycycline, erythromycin, ciprofloxacin, and azithromycin were 97.2%, 5.6%, 11.1%, and 11.1%, respectively. Conclusion: UU was the leading causative pathogen for genitourinary infection in our study. MH accounted for about half of the UU infection rate. Doxycycline was still best susceptible to UU, MU, and both. Ciprofloxacin had less than 20% susceptibility to MU, UU, both. The major problem is that the pregnant women is infected with MU and both are not sensitive to erythromycin and azithromycin at all.       • Two cases of duodenal ulcer perforation in the postpartum period several days after cesarean section

        Small bowel perforation in the postpartum period is very rare and is often misdiagnosed as functional paralytic ileus after surgery or pain at the wound, rapid and accurate diagnosis can be very difficult and delayed for various reasons, which can threaten the life of the mother. In the past 10 years, we have had experienced two cases of duodenal ulcer perforation after cesarean section. In all two cases, there was no past history of the ulcer, the first case was too severe and laparotomy after laparoscopic attempt was performed, and the second case was treated laparoscopic surgery by 3-0 V-LocTM absorbable wound closure device. In case of unexpected abdominal pain, especially in the case of accompanying fever during well recovery 4-7 days after cesarean section, if the surgical site is normal, the gastrointestinal disorder should be considered.       • Secondary amenorrhea by craniopharyngioma with boderline high prolactin

        Most common hypothalamus-pituitary tumor followed by primary or secondary amonorrhea is craniopharygioma which mainly arises form upper sella area and also have one or more neurologic symptoms such as deteriorating vision and severe headache. Decreased GnRH secretion due to stalk compression may result in panhypopituitarism. Unlike other pituitary hormones, prolactin secretion is mainly controlled by potent negative inhibition of hypothalamic dopamine. Large sella masses or traumatic injuries are common causes of hyperprolactinemia of broken dopamine regulation. A 19-year-old woman presented to us in gynecologic out patient department with history of amenorrhea for 14 months. She was a virgin, had no medical history. From the menarche stared 6 years ago, her menstruation cycle had been about two-month till the menstruation stopped a year ago. She said she gained 10kg for a year. Trans-rectal ultrasonography of pelvis showed small uterus 37x20mm, polycystic ovaries. Endometrial thickness was 2mm. On a blood test taken at a local clinic 2 months ago, LH (Luteinizing Hormone) 0.4mIU/mL, FSH (Follicle Stimulating Hormone) 2.0mIU/mL, E2 (Estradiol)<10pg/mL, TSH (Thyroid Stimulating Hormone) 1.0IU/mL, Prolactin 54.3ng/mL were checked. Re-checked prolactin level at our clinic was 58.78ng/mL. Under the diagnosis of hypogonadotropic hypogonadism, clomiphene 50mg for 5 days and carbegoline 0.5mg once in a week were prescribed. Finally the drug was stopped in fifteen months because the prolactin was lowered to 3.62ng/mL despited reducing the dosage to 0.125mg per two weeks. But the prolactin level rose to 122.80ng/mL just in eight weeks after stopping the drug. Because the prolactin level was too high, brain MRI (Magnetic Resonance Imaging) was done immediately, which demonstrated a 31x24x16mm delayed enhancing and cystic lesion in sellar and suprasellar area, probably craniopharyngioma.         Pharmacological Unmasking Microarray Approach-Based Discovery of Novel DNA Methylation Markers for Hepatocellular Carcinoma

        <P>DNA methylation is one of the main epigenetic mechanisms and hypermethylation of CpG islands at tumor suppressor genes switches off these genes. To find novel DNA methylation markers in hepatocellular carcinoma (HCC), we performed pharmacological unmasking (treatment with 5-aza-2'-deoxycytidine or trichostatin A) followed by microarray analysis in HCC cell lines. Of the 239 promoter CpG island loci hypermethylated in HCC cell lines (as revealed by methylation-specific PCR), 221 loci were found to be hypermethylated in HCC or nonneoplastic liver tissues. Thirty-three loci showed a 20% higher methylation frequency in tumors than in adjacent nonneoplastic tissues. Correlation of individual cancer-related methylation markers with clinicopathological features of HCC patients (n = 95) revealed that the number of hypermethylated genes in HCC tumors was higher in older than in younger patients. Univariate and multivariate survival analysis revealed that the <I>HIST1H2AE</I> methylation status is closely correlated with the patient's overall survival (<I>P</I> = 0.022 and <I>P</I> = 0.010, respectively). In conclusion, we identified 221 novel DNA methylation markers for HCC. One promising prognostic marker, <I>HIST1H2AE</I>, should be further validated in the prognostication of HCC patients.</P>

        Identification of Novel Methylation Markers in Hepatocellular Carcinoma using a Methylation Array

        <P>Promoter CpG island hypermethylation has become recognized as an important mechanism for inactivating tumor suppressor genes or tumor-related genes in human cancers of various tissues. Gene inactivation in association with promoter CpG island hypermethylation has been reported to be four times more frequent than genetic changes in human colorectal cancers. Hepatocellular carcinoma is also one of the human cancer types in which aberrant promoter CpG island hypermethylation is frequently found. However, the number of genes identified to date as hypermethylated for hepatocellular carcinoma (HCC) is fewer than that for colorectal cancer or gastric cancer, which can be attributed to fewer attempts to perform genome-wide methylation profiling for HCC. In the present study, we used bead-array technology and coupled methylation-specific PCR to identify new genes showing cancer-specific methylation in HCC. Twenty-four new genes have been identified as hypermethylated at their promoter CpG island loci in a cancer-specific manner. Of these, <I>TNFRSF10C</I>, <I>HOXA9</I>, <I>NPY</I>, and <I>IRF5</I> were frequently hypermethylated in hepatocellular carcinoma tissue samples and their methylation was found to be closely associated with inactivation of gene expression. Further study will be required to elucidate the clinicopathological implications of these newly found DNA methylation markers in hepatocellular carcinoma.</P>

      • Assessment of androgen receptor agonistic/antagonistic effects on 25 chemicals in household applicants by OECD <i>in vitro</i> stably transfected transcriptional activation assays

        <P><B>Abstract</B></P> <P>The aim of this study is to assess the androgen receptor (AR) agonistic/antagonistic effects on various chemicals, which are used in household products including cleaning agents and wetted tissues by <I>in vitro</I> OECD test guideline No. 458 (using AR-EcoScreen™ cell line) and the me-too test method (using 22Rv1cell line), which was adopted as OECD project No. 4.99. All chemicals were not determined as AR agonists. However α-dodecyl-ω-hydroxypoly (oxyethylene) and 3-iodo-2-propynyl butylcarbamate have shown a weak AR antagonistic effects with IC<SUB>50</SUB> values of 2.18 ± 0.12 and 4.26 ± 0.17 μg/ml via binding affinity to AR in only 22Rv1/mouse mammary tumor virus using AR transcriptional activation assay, because of their different cytotoxicity on each applied cell line. This report firstly provides information about agonistic/antagonistic effects against human AR of various chemicals including surfactants and biocides by OECD <I>in vitro</I> stably transfected transcriptional activation assays.
