        The purpose of establishing the circulatory disease surveillance system in Korea is to ensure that the problems of circulatory disease importance are being monitored efficiently and effectively. The goals of circulatory disease surveillance system are to monitor the epidemiological trends of circulatory disease and to evaluate the outcome of health activity for controlling circulatory diseases. Surveillance system are being updated to achieve the needs for the integration of the surveillance and information system, the establishment of data standards, the electronic exchange of data, and changes in the goals of circulatory disease surveillance system to facilitate the response of this system to manage the national health problem effectively. This article provides the target diseases and determinant indicators to be monitored, structure of circulatory disease surveillance system, and many tasks and related activities that should be applied to this system.

        Dietary Patterns and Nutrient Intakes of Individuals with Circulatory Diseases: Ansan-Ansung Cohort Data from the Korean Genome and Epidemiology Study

        Recently, there is growing interest in studying the dietary patterns that affect the risk of circulatory system diseases (CSDs). We investigated the relationship between CSDs and dietary patterns through a follow-up study in Korea (2001-2016). The participants of this study were obtained from the Korean Genome and Epidemiology Study (KoGES). This study was a large community-based cohort study (the Ansan-Ansung areas) conducted to assess the effects of various factors, especially diet, on the onset of chronic diseases among the Korean population aged 40-69 yrs. Baseline data were collected from 2001 to 2002, and follow-up studies were performed every 2 yrs, with over 7 follow-up studies performed (2015-2016). Three dietary patterns were identified using factor analysis: “vegetable and seafood (men)/soup and stew (women)” pattern, “sweet foods and breads-rice cake” pattern, and “multigrain rice and cooked white rice” pattern. None of the dietary patterns were significantly associated with the risk of CSDs in either men or women. Our follow-up study is meaningful as it investigated whether the dietary patterns of individuals according to sex affects the development of CSDs.

        응급의료에서의 의사의 주의의무와 법적책임

        응급환자가 발생하였을 경우에 구급을 위한 의료 인력으로서 환자를 응급의료기관에 이송하기 전 단계에서 환자에 대해서 응급처치를 하는 응급구조사(EMT: emergency medical technician)가 있으며, 환자를 응급의료센터에 이송하면 응급의료센터에는 응급환자의 치료를 전담하는 의사와 응급전문간호사 등을 중심으로 전문구급 의료팀이 구성된다. 구급․구조신고를 받은 119종합상황실과 1339응급의료정보센터에서는 구급차량을 출동시켜 사고현장에 도착하여 응급처치를 시행한 이후에 병원으로 이송하게 되는데, 응급환자를 이송 받은 병원에서는 응급의료시스템을 모두 동원하여 응급환자의 병상과 병력 등에 관한 정보가 불충분하거나 본인 내지 보호자의 의사가 불분명한 경우에는 병상의 중증도와 긴급성에 따라서 우선 환자의 생명을 保持하기 위한 목적으로 신속한 검사와 진단을 통하여 치료를 개시하게 된다. 그러나 응급의료인은 응급환자가 당해 의료기관의 시설 등의 부족으로 적정한 치료를 할 수 없다고 판단될 때에는 지체 없이 환자를 치료가 가능한 다른 의료기관으로 이송하여야 하며(응급의료에 관한 법률 제11조 제1항), 응급환자가 의사결정능력이 없거나, 설명 및 동의절차로 인하여 응급의료가 지체됨으로서 환자의 생명에 위험 내지 심신상의 중대한 장애를 초래할 수 있는 경우를 제외하고 응급환자에게 응급의료의 내용에 대하여 설명하고 동의를 얻는 등의 조치를 취해야 한다(응급의료에 관한 법률 제9조 제1항). 그러나 응급환자가 의사결정능력이 없음에도 법정대리인이 동행하지 않거나 환자와 이해관계가 없는 제3자 등이 환자를 이송했을 경우에, 의사는 동행한 자에게 설명한 후 응급처치를 시행하고, 의사의 의학적 판단에 따라서 응급진료를 행하게 된다(응급의료에 관한 법률 제9조 제2항). 그러나 응급환자와 이해관계가 없는 사람이 환자를 발견하여 의료기관에 이송한 경우에도 의사는 응급환자의 상태를 설명하고 동의를 얻어 응급진료를 행하여야 하는가, 또 응급환자를 응급병원에 이송하였으나 응급의료기관이 진료를 거부하는 경우, 뿐만 아니라 당해 응급의료기관의 능력 등으로 당해 환자의 적정한 치료가 불가능할 경우 다른 의료기관으로의 전원을 위한 판단과 시기, 방법 등과 관련하여 많은 문제가 발생할 수 있다. 이 같은 문제의식을 가지고 본 연구는 의사의 주의의무와 책임을 중심으로 나타날 수 있는 갈등을 사전에 예측하여 검토하였다.

        Korea Heart Disease Fact Sheet 2020: Analysis of Nationwide Data

        Background and Objectives: Cardiovascular disease (CVD) is the leading cause of death and disability worldwide. To provide an overview of the temporal trends in the burden of CVD, the Korean Society of Cardiology has published the Korea Heart Disease Fact Sheet in 2020. Methods: We analyzed anonymized data of the Causes of Death Statistics, National Health Insurance Claims Database, and Korea National Health and Nutrition Examination Survey to assess mortality, hospitalizations, and risk factors for CVD. Results: The CVD mortality decreased until 2010, then steadily increased up to 123 per 100,000 persons in 2018. Since 2002, the number and rate of CVD hospitalization have continued to grow. In 2018, approximately 12.1 million Korean adults had hypertension, 4.3 million had diabetes, 8.7 million had hypercholesterolemia, 14.9 million had obesity, and 8.8 million were currently smoking. The number of risk factors increased markedly with older age; 58.4% of adults age ≥70 years had ≥2 risk factors. Conclusions: CVD mortality and hospitalization have gradually increased in the last decade, and a substantially high proportion of adults were carrying more than 1 cardiovascular risk factor in 2018. With the rapid population aging, a continued increase in CVD appears inevitable in Korea. Concerted and sustained approaches are essential to achieve early prevention and reduce the burden of CVD.
